NEW TECHNOLOGIES IN LAPAROSCOPIC TREATMENT OF ACQUIRED LIVER CYSTS

Laparoscopic ultrasound is a method that combines the principles of intraoperative ultrasound and laparoscopy. Due to the impossibility of palpation during laparoscopic operations, which are often accompanied by inflammatory-infiltrative changes and abnormal vessel and duct anatomy, the use of laparoscopic ultrasound allows for the avoidance of iatrogenies and exploratory laparotomies.

Objective of the study: improving the results of laparoscopic treatment of acquired liver cysts by using intraoperative ultrasound navigation

Clinical case of treatment of a 15-year-old girl. Seven months ago, multiport laparoscopic echinococcectomy SVI-SVIII was performed for a large parasitic cyst (980 ml). According to ultrasound and CT data, there is a residual cavity in the right lobe of the liver SVIII (dimensions 100x72x97 mm, volume 350 cm ^ 3). Studies on the recurrence of liver echinococcus are negative. Multiport laparoscopic cystectomy with intraoperative ultrasound navigation with a miniature Arietta 750SE sensor (Fujifilm, Japan) was performed. A cystic formation measuring 6.0 x 10.0 cm in the right lobe of the liver (SVII-SVIII) was detected. Using an ultrasonic scalpel, the cyst was opened, and about 380 ml of yellow-green fluid was obtained. The fibrously altered cyst capsule was excised with the treatment of the walls of the residual cavity. A safety drainage was installed. The use of the proposed intraoperative complementation ensured a significant reduction in the time of the surgical intervention, prevented the development of intra- and postoperative complications, and ensured the patient's safety in the hospital.
