EARLY SUPRAPUBIC CATHETERIZATION IN COMPLETE URETRA RUPTURE

Aim: Major urogenital injuries in children often occur as part of high-energy trauma accompanied by multiple system injuries. Erectile dysfunction, incontinence and stricture after urethral rupture are serious problems in these patients. In this study, we aimed to discuss the patient with stage 4 urethral trauma and treatment management.

Case: A 5-year-old male patient was admitted to the hospital due to the inability to urinate after a car accident and blood from the urethra. The patient had right ischium, right iliac wing and left ramus pubis fracture. CT showed that the bladder was full and the bladder neck deviated to the right. A complete rupture of the posterior urethra was observed at urethrography and then suprapubic catheter was inserted. The patient's hematocrit was stable and cystoscopy was performed, but the proximal part of the rupture could not be achieved. The urethra was catheterized from the suprapubic catheter tract with cystoscopy and a foley catheter was inserted through the catheter. Postoperative sixth week foley was removed and suprapubic catheter was removed at the 8th week. During follow-up, the patient had no complaints and urinary USG and urethrography were normal. Uroflowmetry was show that Qmax was normal.

Conclusion: Early urethral catheterization may be a good alternative with low complication results compared to late repair after cystofix and emergency primary repair.

KOMPLET ÜRETRA KOPMASINDA ERKEN SUPRAPUBİK KATETERİZASYON

Amaç: Çocuklarda majör ürogenital yaralanmalar çoğunlukla çoklu sistem yaralanmalarının eşlik ettiği yüksek enerjili travmaların bir parçası olarak meydana gelir. Üretra kopması sonrası erektil disfonksiyon, inkontinans ve striktür bu hastalarda karşılaşılan ciddi problemlerdir. Bu çalışmada evre 4 üretra travması olan hastayı ve tedavi yönetimini tartışmayı amaçladık.

Olgu: 5 yaşında erkek hasta, araç dışı trafik kazası sonrası idrar yapamama ve üretral meadan kan gelmesi nedeniyle yatırıldı. Yapılan tetkiklerde hastanın sağ iskium, sağ iliak kanat ve sol ramus pubis fraktürü mevcuttu. Tomografide mesanenin dolu olduğu ve mesane boynunun sağa doğru deviye olduğu görüldü. Üretrografide posterior üretrada komplet rüptür görüldü ve sistofiks takıldı. Hematokriti stabil olan hastaya sistoskopi yapıldı fakat rüptürün proksimaline geçilemedi. Sistofiks traktından suprapubik olarak sistoskopi eşliğinde üretra kateterize edildi ve kateter üzerinden foley sonda yerleştirildi. Post op 6. hafta foley, 8. hafta sistofiks çekildi. Takiplerinde hastanın şikayeti olmayıp üriner USG ve üretrografisi normaldi. Hastaya üroflowmetri çekildi ve Qmax normaldi.

Sonuç: Erken üretral kateterizasyon, sistofiks sonrası geç onarım ve acil primer onarıma düşük komplikasyon sonuçlarıyla iyi bir alternatif olabilir.
