THE APPROACH OF PEDIATRIC SURGEONS AND PEDIATRIC UROLOGOISTS TO ENDOSCOPIC TREATMENT OF VESICOURETERAL REFLUX DISEASE

Introduction and purpose

Vesicoureteral reflux (VUR) is the most common cause of pyelonephritis in childhood. Reflux degree and symptoms reveal the treatment algorithm. In general, VUR treatment can be either conservative or interventional and may include continuous antibiotic prophylaxis, endoscopic treatment, or antireflux surgery (open or laparoscopic).

The pediatric surgeons and pediatric urologists were asked about the endoscopic treatment of VUR and their preferences about the controversial issues. After receiving the answers, the variables in endoscopic management were determined.

Materials and methods

The electronically prepared questionnaire was sent to the electronic mail and social media in Turkey Pediatric Surgeon and Pediatric Urologists. The answers to the questions were recorded in our electronic account. The answers were categorically divided into percentages.

Results and discussion

Most of the answers to the questionnaire questions were answered by doctors in university hospitals. Endoscopic treatment, decisions about VUR surgery may change, it was mostly treated with third-degree reflux and a maximum of 2 injections was performed. The dextranomer hyaluronic acid copolymer which is the injection material was preferred. Dextranomer hyaluronic acid copolymer was reported to be more effective and safe. The most common STING technique was preferred.

The endoscopic treatment and the responses in the injection procedure were similar, but there were different approaches to follow-up after endoscopic treatment. After endoscopic treatment, different approaches are developed for treated patients.

ÇOCUK CERRAHLARI VE ÇOCUK ÜROLOGLARININ VEZİKOÜRETERAL REFLÜNÜN ENDOSKOPİK TEDAVİSİNE YAKLAŞIMLARI

Giriş ve Amaç

Veziko-üreteral reflü (VUR) çocukluk çağında pyelonefritlerin en sık nedenidir. Semptomlar ve reflü derecesi tedavi endikasyonunu belirler. Genellikle konzervatif veya girişimsel tedaviler tercih edilr. Bunlar antibiyotik proflaksisi, endoskopik tedavi, antireflü cerrahisi (açık veya laparoskopik) gibi tedavi yöntemleridir.

Çocuk cerrahları ve çocuk ürologlarına VUR’un endoskopik tedavisi ile ilgili parametrelerde ve tartışmalı konulardaki tercihleri soruldu. Cevaplar üzerinden endoskopik tedavi yaklaşımlarındaki farklılıklar ortaya konuldu.

Materyal ve metod

Elektronik ortamda hazırlanan anket formu, Türkiye genelindeki Çocuk Cerrahları ve Çocuk Üroloji Uzmanı hekimlere elektronik mail ve sosyal medya hesapları üzerinden ulaştırıldı. Sorulara verilen cevaplar elektronik ortama kayıt edildi. Bu kayıtlar kategorik olarak yüzdelik dilimlere bölündü.

Sonuçlar ve tartışma

Çalışmaya katılanlar daha çok üniversite hastanelerinde çalışıyordu. Endoskopik tedavinin VUR’a müdahale kararlarını etkilediği, daha çok 3.ü derece reflülere endoskopik tedavi yapıldığı ve en fazla 2 defa enjeksiyon yapıldığı belirtilmiş. Enjeksiyon materyali olarak dextronomer hyaluronik asit kopolimeri tercih edilmiş. Aynı materyalin daha etkili ve güvenli olduğu belirtilmiş. En sık STING tekniği uygulandığı belirtilmiş.

Endoskopik tedavi öncesi uygulamalar ve enjeksiyon işleminde verilen cevaplar genel olarak benzerdi, fakat endoskopik tedavi sonrası takip ile ilgili farklı yaklaşımlar söz konusu. Endoskopik tedavi sonrası her hastada farklı yaklaşımlar olabilmektedir.
