Testicular Epidermal Cyst

Aim:
Testis tumors are rare in children. Most of these tumors are malignant, and only less than 1% are benign. In this study, a testicular epidermal cyst case is reported, and testicular lesions are aimed to discuss.
Case Report:
A 14 year old boy who was followed by pediatric endocrinology clinic for obesity was found to have a palpable mass in left testis. Hematological and biochemical serum tests and tumor marker (Alpha Feto Protein and beta HcG) levels were within normal limits. Ultrasonography revealed a heterogenous iso- and hypoechoic mass located inferolaterally in lower pole of left testis sized 18x13x19 in millimeters, without an internal vascularity. The patient was operated through an inguinal incision, and 3 white colored, hard, and smooth bordered intratesticular lesions were totally excised by testis sparing surgery. The masses and neighbouring testicular tissue was frozen section examined perioperatively. The pathological examination of specimens were reported as epidermal cyst with surrounding normal testicular tissue. The capsule of the testis was repaired, and left testis was placed back in scrotum.
Conclusion:
Frozen section examination of testicular masses seems as an instructive approach for testis sparing surgery if tumor marker levels are normal and have smooth borders without any invasion of surrounding tissue in radiological evaluation.
