Introduction: In patients with penoscrotal hypospadias, repair of the urethral chordee in the first session, applying a flap or graft to create the urethral plate, and tubularizing the urethral plate in the second session, is a common approach to repair in two sessions. Traditionally, a minimum of 6 months is expected between two sessions. In our study, it was aimed to compare the results of patients who had 6 months to 1 year or more between the two sessions.
Method: It is planned prospectively to include patients who underwent two-session repair between 2018 and 2022. The demographic characteristics of the patients, the time between the two sessions and the complications that developed after the second session were recorded. Patients with a period of 6 months between two sessions were included in one group (Group A: 11 patients), and patients with a duration of one year or more were included in the other group (Group B: 10 patients).
Results: The mean age was 27.6 months in group A and 20.5 months in group B. There was no significant difference between them (p= 0.314). Complications were seen in 4 patients in the 6-month waiting group. Complications were seen in 1 patient in the group expected for 1 year or more. This difference was not statistically significant (p=0.157). In Group A, two patients had stenosis, one patient had urethral fistula, and one patient had dilation of the glans. In Group B, stenosis was detected in one patient. The average time between two sessions was 19.7 months (12-32 months) in patients who were expected for one year or more.
Conclusion: When two-session surgery is planned for penoscrotal hypospadias, waiting one year or more between two sessions may reduce the complication rate. In our study, a difference was observed between the results of patients with a period of 6 months between the two sessions and a period of one year or more, but this difference did not reach a statistically significant level. Prospective studies with larger numbers of patients are needed.

Giriş: Penoskrotal hypospadiaslı hastalarda ilk seansta üretral kordi onarımı yapılıp Üretral plate oluşturmak için flep veya greft uygulanması ve ikinci seansta Üretral plate’in tübülarize edilmesi ile iki seans onarımı sıklıkla uygulanan bir yaklaşımdır. İki seans arasında geleneksel olarak en az 6 ay süre beklenmektedir. Bizim çalışmamızda iki seans arasında 6 ay ile bir yıl ve üzeri zaman olan hastaların sonuçlarının karşılaştırılması amaçlanmıştır.
Yöntem: 2018-2022 yılları arasında iki seanslı onarım yapılan hastaların çalışmaya alınması prospektif olarak planlanmıştır. Hastaların demografik özellikleri, iki seans arasında geçen süre ve ikinci seanstan sonra gelişen komplikasyonlar kaydedildi. İki seans arasında 6 ay olan hastalar bir gruba (Grup A: 11 hasta), bir yıl ve daha fazla süre olan hastalar diğer gruba (Grup B: 10 hasta) alındı.
Bulgular: Grup A’da yaş ortalaması ortamalası 27,6 ay, grup B’de 20.5 ay idi. Aralarında anlamlı fark yoktu( p= 0.314). 6 ay beklenen grupta 4 hastada komplikasyon görüldü. 1 yıl ve üzeri beklenen grupta 1 hastada komplikasyon görüldü. Bu fark istatistik olarak anlamlı değildi (p=0.157). Grub A’da iki hastada darlık, bir hastada üretral fistül ve bir hastada glans açılması görüldü. Grup B’de ise bir hastada darlık saptandı. Bir yıl ve üzeri beklenen hastalarda iki seans arasındaki süre ortalama 19,7 ay ( 12-32 ay) idi.
Sonuç: Penoskrotal hipospadiasta iki seanslı cerrahi planlandığında iki seans arasında bir yıl ve üzeri zaman beklenmesi komplikasyon oranını azaltabilir. Bizim çalışmamızda, iki seans arasında 6 ay olan ve bir yıl ve üzeri süre olan hastaların sonuçları arasında fark görülmüş olup bu fark istatistik olarak anlamlı seviyeye ulaşmamıştır. Daha geniş hasta sayısı olan prospektif çalışmalar gerekmektedir.
