Hypertrophic Pyloric Stenosis in Premature Infants: Evaluation of Sonographic Criteria

Aim:Hypertrophic pyloric stenosis (HPS) is the most common cause of non-bilious vomiting in infancy.There are few studies in the literature evaluating sonographic criteria in premature HPS.It was aimed to evaluate HPS criterias in premature infants.

Methods:Patients who were operated for HPS between 2012 and 2022 were included.Gestational histories, clinical findings, admission weights, sonographic HPS criteria of premature and term cases were compared.

Results:A total of 36 cases were operated for PS. 29 cases (F:5,M:24) with complete data were included.The mean gestational week of the cases was 37.5±0.4 weeks, and the mean admission weight was 3411±185 grams.All of the cases had complaints of malnutrition and non-bilious vomiting.Weight percentile loss was detected in 51.7% of the cases. 33.3% of the cases were premature.There was no difference between term cases in terms of age at operation (p=0.236) and duration of symptoms (p=0.737) in premature cases.In addition, when sonographic findings of premature cases were compared with those of term cases, no difference was found between the mean pyloric canal length (p=0.955) and pyloric single wall thickness (p=0.361). Pyloric canal length, which is incompatible with the HPS criterion, was found in 21% of the cases; Incompatible pyloric single wall thickness was detected in 13.8% of the cases. There was no significant correlation between prematurity with the incidence of incompatible canal length (p=0.663) and wall thickness (p=1,000). Although the patients with inconsistent canal length and wall thickness were lower weight at admission, there was no statistical difference (p=0.118 and p=0.339).

Conclusion:In our study, when premature cases with HPS and term cases were compared, it was shown that there was no difference between operative ages and symptom duration.Also sonography measurements were not affected by the weight and gestational week of the case.

Prematür İnfantlarda Hipertrofik Pilor Stenozu: Ultrasonografik Kriterlerin Gözden Geçirilmesi

Amaç: Hipertrofik pilor stenozu (HPS) infant döneminde safrasız kusmanın en yaygın cerrahi nedenidir. Literatürde prematür HPS’larda sonografi kriterlerinin değerlendirildiği az sayıda çalışma mevcuttur. Çalışmamızda prematür infantlarda sonografik HPS kriterlerinin değerlendirilmesi amaçlanmıştır.

Yöntem: Kliniğimizde 2012-2022 yılları arasında HPS nedeniyle opere edilmiş olgular çalışmaya dahil edildi. Prematür ve term olguların gestasyonel öyküleri, klinik bulguları, başvuru ağrılıkları ve sonografik HPS kriterlerinin karşılaştırıldı.

Bulgular: Toplam 36 olgu PS nedeniyle opere edilmişti. Verileri eksiksiz olan 29 olgu (K:5,E:24) çalışmaya dahil edildi. Olguların ortalama gestasyonel haftası 37,5±0,4 hafta, ortalama başvuru kilosu 3411±185 gramdı. Olguların tamamında beslenememe ve safrasız kusma şikayetleri mevcuttu, %51,7’sinde başvurusunda kilo persentil kaybı saptanmıştı. Olguların %33,3’ü prematür doğumdu. Prematür olgularda operasyon yaşı (p=0,236) ve semptom süresi (p=0,737) bakımından term olgulara göre farklılık bulunmadı. Ayrıca prematür olguların sonografi bulguları ile term olgularınki karşılaştırıldığında, ortalama pilor kanal uzunluğu (p=0,955), pilor tek duvar kalınlığı (p=0,361) gruplar arasında farklılık saptanmadı. HPS kriteri ile uyumsuz olan pilor kanal uzunluğu olguların %21’inde; uyumsuz pilor tek duvar kalınlığı olguların %13,8’inde saptandı. Uyumsuz kanal uzunluğu (p=0,663) ve uyumsuz duvar kalınlığı (p=1,000) görülme sıklıkları ile prematürite arasında anlamlı ilişki bulunmadı. Uyumsuz kanal uzunluğu ve duvar kalınlığı olan olguların başvuru kilosu olmayanlara göre daha düşük olmakla beraber, istatistiksel bir farklılık bulunmadı (p=0,118 ve p=0,339).

Sonuç: Çalışmamızda HPS'li prematüre olgular ile term olgular karşılaştırıldığında operasyon yaşları ve semptom süreleri arasında farklılık bulunmadığı ve sonografi ölçümlerinin olgunun kilosu ve gestasyonel haftasından etkilenmediği gösterilmiştir.
