Evaluation of the Systemic Immuno-Inflammation Index and the Systemic Inflammatory Response Index as New Markers in the Diagnosis of Acute Appendicitis in Children

Introduction:

It is very important to identify children with appendicitis early in the course of the disease and to provide timely treatment to prevent serious negative consequences such as sepsis or death. In this study, we aimed to investigate the importance of systemic immune inflammation index (SII), systemic inflammation response index (SIRI) and other systemic inflammatory response blood cell indices in predicting the diagnosis and prognosis of acute appendicitis in children.

Materials and Methods:

The files of 1265 patients who were operated for appendicitis in our clinic between January 2011 and January 2022 were reviewed retrospectively. The demographic data of the patients, their complaints at the time of admission, examination findings, results of imaging tests, complete blood cell count results and pathology results were recorded. According to the pathology results, patients; Patients with pathological findings of appendicitis and those without pathological findings and patients whose pathology results are compatible with appendicitis; They were divided into two groups as perforated appendicitis and acute appendicitis. Systemic inflammation markers, complaints and physical examination findings at admission were statistically compared between the groups.

Results:

The mean age and gender distribution of patients were similar between study groups (p=.453 and p=.124, respectively). Pain (p=.184), nausea (p=.554), rebound (p=162), and abdominal defense (p=.099) symptoms were similar in the study groups. SIRI (7.60 [IQR, 3.69 – 13.97] vs 3.20 [IQR, 1.50 – 6.51], p<0.001) and SII (2489.47 [IQR, 1293.04 – 4169.90] and 1176.98 [IQR, 677.04 – 2395.16], p < .001 was significantly higher in the acute appendicitis group than in the non-appendicitis group. Neutrophil count and percentage, monocytes count, Neutrophil Lymphocyte ratio, Platelet Lymphocyte ratio were found to be significantly higher in the acute appendicitis group compared to the non-appendicitis group.

Conclusions:

To our knowledge, this is the first study to demonstrate the joint diagnostic value of SII and SIRI in children with Acute Appendicitis. We think that SII and SIRI can be used as new markers to support the diagnosis of acute appendicitis in children.

Çocuklarda Akut Apandisit Tanısında Yeni Belirteçler Olarak Sistemik İmmün-İnflamasyon İndeksi ve Sistemik İnflamasyon Yanıtı İndeksinin Değerlendirilmesi

Giriş:

Apandisitli çocukların hastalık seyrinde erken dönemde belirlenmesi ve zamanında tedavi sağlanması sepsis veya ölüm gibi ciddi olumsuz sonuçlardan korunmak için çok önemlidir. Bu araştırmada, hastanemize başvuru anındaki şikayet ve muayene bulguları ile sistemik immün enflamasyon indeksi (SII), sistemik inflamasyon yanıt indeksi (SIRI) ve diğer sistemik inflamatuar yanıtın kan hücresi indekslerinin çocuklarda akut apandisit tanısını ve prognozunu tahmin etmedeki önemini araştırmayı amaçladık .

Materyal ve Metod:

Ocak 2011- OCAK 2022 yılları arasında kliniğimizde apandisit nedeniyle ameliyat edilen 1265 hastanın dosyaları retrospektif olarak incelendi. Hastaların demografik verileri ile başvuru anındaki şikayetleri, muayene bulguları, görüntüleme tetkiklerinin sonuçları, tam kan hücresi sayımı sonuçları ve patoloji sonuçları kayıt altına alındı. Patoloji sonuçlarına göre hastalar; apandisitin patolojik bulguları olanlar ve olamayanlar şeklinde iki gruba ve patoloji sonuçları apandisit ile uyumlu olan hastalar ise ; perfore apandisit ve akut apandisit şeklinde iki gruba ayrıldı. Gruplar arasında sistemik inflamasyon belirteçleri ile başvuru anındaki şikayet ve fizik muayene bulguları istatistiksel olarak karşılaştırıldı.

Bulgular:

Hastaların ortalama yaş ve cinsiyet dağılımı çalışma grupları arasında benzerdi (sırasıyla p=.453 ve p=.124). Çalışma gruplarında ağrı (p=.184), bulantı (p=.554), rebound (p=162) ve abdominal defans (p=.099) semptomları benzerdi. SIRI (7,60 [IQR, 3,69 – 13,97] vs 3,20 [IQR, 1,50 – 6,51], p<0,001) ve SII (2489,47 [IQR, 1293.04 – 4169.90] ve 1176.98 [IQR, 677.04 – 2395.16], p <.001 akut apandisit grubunda apandisit olmayan gruba göre anlamlı olarak daha yüksekti. Akut apandisit grubunda nötrofil sayısı ve yüzdesi, monosit sayısı, Nötrofil Lenfosit oranı, Platelet Lenfosit oranı değerleri apandisit olmayan gruba göre anlamlı olarak yüksek bulundu.

Sonuç:

Bildiğimiz kadarıyla bu çalışma Akut Apandisitli çocuklarda SII ve SIRI’ nin ortak tanısal değerini gösteren ilk çalışmadır. SII ve SIRI’ nin çocuklarda akut apandisit tanısını destekleyen yeni belirteçler olarak kullanılabileceğini düşünüyoruz.
