Bilateral cystic nephroma

Introduction: Cystic nephroma (CN) is a rare, asymptomatic, benign kidney tumor. Clinically and radiologically, differentiation of CN from malignant tumors such as cystic partially differentiated nephroblastoma and cystic Wilms tumor is difficult, and the definitive diagnosis is made by histopathological examination. In addition, bilateral involvement and familial cases associated with a mutation in the DICER-1 gene have been reported. The effective method in the treatment of CN is the removal of the tumor.

Case: A fourteen-month-old male patient was brought by his family because of noticing abdominal distension and bloody urine in his diaper. On examination, the abdomen was distant. A palpable mass of approximately 5 x 5 cm was palpated in the bilateral kidney lodge. In the abdominal ultrasonography, a cystic mass of approximately 10 x 6 cm in the upper half of the right kidney and 11 x 6.6 cm in the left kidney, consisting of multiple cysts separated from each other by multiple thin septa and causing significant suppression and thinning in the adjacent parenchyma, was observed. A clear radiological distinction could not be made for the identification of the cyst in upper abdominal magnetic resonance and positron emission tomography. Cystic nephroma or cystic nephroblastoma were considered. Left nephrectomy was performed because the echogenicity and thickness of the parenchyma of the right kidney were observed in the lower half of the radiological studies. Pathology result came as cystic nephroma. In the follow-up, there was no deterioration in the functions of the right kidney, and no growth in the size of the cyst was detected radiologically.

Conclusion: Bilateral cystic nephroma is very rare in the literature. After performing a left-sided nephrectomy, DICER-1 genetic test was requested and the follow-up was maintained, since there was no growth in the mass in the right-sided follow-ups.

Bilateral kistik nefroma

Giriş: Kistik nefroma (KN) nadir görülen, asemptomatik, iyi huylu bir böbrek tümörüdür. Klinik ve radyolojik olarak KN’nın, malign tümörlerden kistik parsiyel farklılaşmış nefroblastom ve kistik wilms tümöründen ayrımı zordur. Kesin tanı histopatolojik inceleme ile konulur. Ayrıca DICER-1 genindeki bir mutasyonla ilişkili olan bilateral tutulum ve ailesel vakalar bildirilmiştir. KN tedavisinde etkin yöntem tümörün çıkarılmasıdır.

Olgu: On dört aylık erkek olgu ailesi tarafından karında şişlik farkedilmesi, bezinde kanlı idrar görülmesi nedeni ile getirildi. Muayenesinde batın distandüydü. İki taraflı böbrek lojunda yaklaşık 5 x 5 cm ele gelen kitle palpe edildi. Batın ultrasonografisinde sağ böbrek üst yarısında yaklaşık 10 x 6 cm, sol böbrekte de 11 x 6,6 cm boyutunda, multiple ince septalar ile birbirinden ayrılan multiple kistlerden oluşan ve komşu parankimde belirgin baskılanma ve incelmeye neden olan kistik kitle izlendi. Üst abdominal manyetik rezonans, pozitron emisyon tomografilerinde de kistin tanınmasına yönelik net bir radyolojik ayrım yapılamadı. Kistik nefroma veya kistik nefroblastoma düşünüldü. Radyolojik çalışmalarda sağ böbreğin parankim ekojenitesi ve kalınlığı izlenebilen alt yarısında olağan izlendiği için sol nefrektomi yapıldı. Patoloji sonucu kistik nefroma olarak geldi. Takipte sağ böbreğinin fonksiyonlarında bozulma olmadı, radyolojik olarak kistin boyutunda büyüme saptanmadı.

Sonuç: Bilateral kistik nefroma literatürde oldukça nadir görülür. Sol tarafa nefroktomi yaptıktan sonra sağ tarafa yönelik takiplerde kitlede büyüme olmaması nedeniyle DICER-1 genetik test istenerek takipte kalınıldı.
