PEDIATRIC MALIGNANT OVARIAN TUMORS

Objective: Presentation the clinical experience of patients diagnosed with malignant ovarian tumor

Methods: Patients diagnosed with malignant ovarian tumor, including 2007-2021, were retrospectively reviewed. Age, symptoms, pre- and postoperative tumor markers, tumor stages, surgical methods, recurrences and survivals were evaluated.

Results: 12 patients included with a mean age of 13.7±2.7 (7.2-16.7) years. The complaints on presentation were abdominal pain (n=6), abdominal distension (n=3), fever and vomiting (n=1), hair growth and deep voice (n=1), dyspepsia and constipation (n=1). 7 patients had a right tumor and 5 left. 11 patients had laparotomy and 1 patient who detected tumor rupture had laparoscopy started open surgery. Torsion was present in 2 patients. Oophorectomy was performed in 3 patients, and salpingo-oophorectomy was performed in 9 patients. In 1 patient, diagnosis was made by wedge biopsy in second ovarian detorsion surgery. Salpingo-oophorectomy was performed later. diagnosis Germ cell tumors in 9 patients (75%), Leydig cell tumors in 2 (17%), and granulosa cell tumors in 1 (8%) are diagnosed in histopathology. Of the 9 patients whose preoperative blood tests were available, 3 had high alpha-fetoprotein (AFP), high beta-HCG in 3 of 7 patients, and high Ca 125 in 7 of 8 patients. According to FIGO staging; 7 patients were stage 1, 1 patient was stage 2, and 2 patients were stage 4. 2 patients could not be staged. 6 of 10 patients, received chemotherapy. Recurrence was observed 4 months later in a stage 4 patient with peroperative tumor rupture, and radical surgery was performed. 11 patients were alive with a mean follow-up of 91.5 (19.3-172.2) months. One patient with a diagnosis of dysgerminoma whose AFP elevation persisted after the operation died 2 years after the operation.

Conclusion: Malignant masses of the ovary are rare in children. These tumors are histopathologically heterogeneous. The survival rate is high in patients who are diagnosed at an early stage and without any rupture-like complications.

PEDİATRİK MALİGN OVER TÜMÖRLERİ

Amaç: Malign over tümörü tanısı alan hastalara yönelik klinik deneyimin paylaşılması

Yöntem: 2007-2021 yılları dahil malign over tümörü tanısı alan hastalar geriye dönük tarandı. Başvuru yaşı, semptomları, ameliyat öncesi ve sonrası tümör belirteç seviyeleri, tanı anında tümör evresi, ameliyat yöntemleri, nüks oranı ve sağ kalım oranı incelendi.

Bulgular: Yaş ortalaması 13,76±2,78 (7,29-16,75) yıl olan 12 hasta vardı. Başvuru yakınmaları karın ağrısı (n=6), karında şişlik (n=3), ateş ve kusma (n=1) kıllanma artışı ve ses kalınlığı (n=1), dispepsi ve kabızlıktı (n=1). Hastaların 7’sinde sağ, 5’inde sol overde tümör vardı. 11 hastaya laparatomi yapıldı; 1 hastada laparoskopik başlandı, tümör rüptürü saptandı ve laparatomi yapıldı. 2 hastada torsiyon mevcuttu. 3 hastada ooferektomi, 9 hastada salfingo-ooferektomi uygulandı. Bir hastada ikinci over detorsiyonu ameliyatı sırasında kama biyopsi alınarak tanı kondu. Farklı seansta salfingo-ooferektomi uygulandı. Histopatolojik tanıda 9 hastada (%75) germ hücreli tümör, 2 hastada (%17) Leyding hücreli tümör ve 1 hastada (%8) granüloza hücreli tümör saptandı. Germ hücreli tümörlerin 4’ü disgerminom, 3’ü immatür teratom, 1’i mikst tip ve1’i yolk kesesi tümörüydü. Ameliyat öncesi kan tetkiklerine ulaşılabilen 9 hastanın 3’ünde alfa-fetoprotein (AFP) yüksekliği, 7 hastadan 3’ünde beta-HCG yüksekliği, 8 hastanın 7’sinde Ca 125 yüksekliği vardı. FİGO evrelemesine göre; 7 hasta evre 1, 1 hasta evre 2, 2 hasta evre 4 idi. 2 hastanın evrelemesi yapılamadı. 10 hastadan 6’sı kemoterapi aldı. Peroperatif tümör rüptürü olan evre 4 tanılı hastada 4 ay sonra nüks görüldü, hastaya radikal cerrahi yapıldı. 11 hasta ameliyat sonrası ortalama 91.5 (19.3-172.2) ay takibinde sağdır. Ameliyat sonrası AFP yüksekliği devam eden disgerminom tanılı 1 hasta ameliyattan 2 yıl sonra eksitus oldu.

Sonuç: Çocuklarda overin malign kitleleri nadir görülür. Bu tümörler histopatolojik olarak heterojen yelpazede yer almaktadır. Erken evrede ve rüptür benzeri komplikasyon gelişmeden tanı alan hastalarda sağkalım oranı yüksektir.
