A case report of traumatic penile fracture in a 15-year-old male patient

Abstract

Purpose: Penile fracture is an acute pathology that can develop as a result of impact or falling on the erect penis during sexual intercourse, masturbation or an erection. Although it is usually seen in adolescence in pediatric patients, it is mostly seen during sexual intercourse and it accompanies pelvic bone fractures in major traumas and penile fracture in urethral injuries. We aimed to present the treatment and follow-up approaches in a 15-year-old male patient who developed penile fracture after blunt trauma.

Case Report: The patient, who applied to the emergency department with the complaints of severe pain, ecchymosis and oedema in the penis and had a history of blunt trauma to the penis the previous day, was diagnosed with penile fracture in the first examination and underwent surgery. In the operation, the urethra was catheterized and it was observed that the tissue integrity of the corpus cavernosum was impaired after degloving and there was no urethral injury. The corpus cavernosum was repaired and then an artificial erection was achieved.

Conclusion: The patient was discharged on the thirteenth post-operative day, and no complications developed in the follow-up. In the post-discharge controls, urine output was in normal calibration and no cosmetic problem was encountered.

15 yaş erkek hastada travmatik penil fraktür olgu sunumu

Özet

Amaç: Penil fraktür cinsel ilişki, mastürbasyon esnasında veya ereksiyonda olan penise darbe veya üzerine düşülmesi sonucu gelişebilen, tunika albugineanın hasar görmesi sonrası peniste şiddetli ağrı , ekimoz , ödem gibi semptomlar geliştiği akut bir patolojidir. Çocuk hastalarda genellikle ergenlik çağında görülmekle birlikte en çok cinsel ilişki sırasında görülmüş olup major travmalarda pelvik kemik kırıklarıyla beraber üretral yaralanmalarda penil fraktüre eşlik etmektedir. 15 yaş erkek hastada künt travma sonrası penil fraktür gelişen hastamızda tedavi ve takip yaklaşımlarını sunmayı amaçladık

Olgu Sunumu: Acil servise peniste şiddetli ağrı , morarma, şişlik şikayetiyle başvuran ve önceki gün penise künt travma öyküsü olan hastanın yapılan ilk muayene ve tetkiklerinde penil fraktür tanısı konularak operasyona alındı. Operasyonda üretra kateterize edildi ve degloving sonrası korpus kavernozumun doku bütünlüğünün bozulduğu ve üretral yaralanma olmadığı gözlendi. Korpus kavernozum onarıldı ve sonrasında artifisiyel ereksiyon sağlandı.

Sonuç: Operasyon sonrası onüçüncü günde taburcu olan hastanın takiplerinde komplikasyon gelişmedi. Taburculuk sonrası kontrollerinde normal kalibrasyonda idrar çıkışı olup kozmetik bir problemle karşılaşılmadı.
