Is routine concomittant antireflux surgery justified at the time of percutaneous endoscopic gastrostomy? A-16-year single center experience

Aim: It was aimed to determine the need for antireflux surgery in the follow-up of patients who had only percutaneous endoscopic gastrostomy (PEG).

Method: The files of the years 2006-2021 were scanned.PEG, gastroesophageal reflux (GER) tests and post-PEG antireflux surgery were evaluated. Patients who underwent PEG during or after antireflux surgery were excluded.

Results:There were 321 patients with a median age of 3(19 days -18 years) years.188(58%) were boys and 133(42%) were girls.Primary diagnosis, chronic neurological disease (n=189), metabolic disease (n=44), traumatic brain injury (n=36), oncological disease (n=19), cardiac malformation (n=11), muscle disease (n= 9), chronic renal failure (n=8) and cleft palate (n=5).The remaining 121 patients underwent 24-hour pH monitoring (n=107), GER scintigraphy (n=35), OMD(n=35), and/or pH impedance (n=1). While only 1 examination was performed in 73 patients, 2 examinations were performed in 39 patients and 3 examinations in 9 patients due to strong suspicion. 224 patients were followed up for a mean of 60.5(6weeks-142months)months.97 patients were unfollowed.127 patients were alive, 182 patients died, and the survival information of 12 patients could not be obtained. An average of 7.5 (1-14) months after PEG, 4 patients underwent antireflux surgery.In 3 of them, tests for GER before PEG were negative.Antireflux surgery was performed due to persistent vomiting in two patients and frequent aspiration pneumonia in one patient.PEG was applied to the other patient while he was lying in the intensive care unit, he could not tolerate feeding and antireflux surgery was performed in the follow-up.

Conclusion:There is an opinion advocating of antireflux surgery in the same session, especially in patients who underwent gastrostomy due to chronic neurological disease-like problems. The clinical questioning and examination for reflux showed that most patients (98.3%) didn’t require simultaneous antireflux surgery with gastrostomy.It is important to keep surgical trauma to a minimum in this patient group.

Perkütan endoskopik gastrostomi ile eşzamanlı rutin antireflü cerrahi yapılmalı mıdır? : 16 yıllık tek merkez deneyimi

Amaç: Beslenme güçlüğü nedeniyle yalnızca perkütan endoskopik gastrostomi (PEG) uygulanan hastaların, takiplerinde antireflü cerrahi ihtiyacının belirlenmesi amaçlandı.

Yöntem: 2006-2021 yıllarına ait dosya kayıtları tarandı. PEG uygulama endikasyonları, girişim öncesi gastroözofageal reflüye (GÖR) yönelik tetkikler ve sonuçları, PEG sonrası antireflü cerrahi uygulamaları değerlendirildi. Antireflü cerrahi sırasında veya sonrasında PEG uygulanan hastalar dışlandı.

Bulgular: Ortanca yaşı 3 (19 gün -18 yıl) yıl olan 321 hasta vardı.188’i (%58) erkek ve 133’ü (%42)kızdı.Primer tanı, kronik nörolojik hastalık (n=189), metabolik hastalık (n=44), travmatik beyin hasarı (n=36), onkolojik hastalık (n=19),kardiyak malformasyon (n=11), kas hastalığı (n=9), kronik böbrek yetmezliği (n=8) ve yarık damaktı (n=5).Ayrıntılı klinik sorgulama ile 200(%62) hastada PEG öncesi GÖR’e yönelik tetkik yapılmasına gerek görülmedi. Kalan 121 hastada 24 saat’lik pH monitorizasyonu (n=107), GÖR sintigrafisi (n=35), ÖMD (n=35) ve/veya pH impedans (n=1) yapıldı.73 hastada yalnızca 1 tetkik yapılırken, kuvvetli şüphe nedeniyle, 39 hastada 2 tetkik ve 9 hastada 3 tetkik yapıldı.224 hasta ortalama 60,5 (6hafta-142ay) ay takip edildi. 97 hasta takipsizdi. Bu çalışmanın yapıldığı tarihte, 127 hasta sağdı, 182 hasta eksitus olmuştu ve 12 hastanın sağ kalım bilgisine ulaşılamadı.PEG uygulanmasından ortalama 7,5 (1-14) ay sonra 4 hastaya antireflü cerrahi yapıldı.Bunların 3’ünde PEG öncesi GÖR’e yönelik tetkikler negatifti.İki hastada sürekli kusma ve 1’inde sık aspirasyon pnömonisi nedeniyle antireflü cerrahi uygulandı.Diğer hastaya PEG uygulaması yoğun bakım ünitesinde yatarken yapılmıştı, takibinde beslenmeyi tolere edemedi ve antireflü cerrahi yapıldı.

Sonuç: Özellikle kronik nörolojik hastalık benzeri süreğen sorunlar nedeniyle gastrostomi uygulanan hastalardan aynı seansta antireflü cerrahi uygulanmasını savunan bir görüş vardır. Bu yaklaşım tartışmaya açıktır. Yeterli klinik sorgulama ve seçilmiş hastalarda reflüye yönelik tetkik yapılarak, çoğu hastada (%98,3) gastrostomi ile eş zamanlı antireflü cerrahi yapılmasına gerek olmadığı görülmüştür. Çoğu yüksek morbidite ve mortaliteye sahip bu hasta grubunda cerrahi travmanın en azda tutulması önemlidir.
