Renal Cell Carcinoma in Postoperative and Long-Term Follow-up in a Patient Receiving Neuroblastoma Treatment, A Case Report

Introduction: Though its pathophysiology not clear, there are similar case reports of the occurrence of renal cell carcinoma (RCC) after neuroblastoma in literature. Genetic mutations, chemotherapy and radiotherapy performed for the treatment of neuroblastoma are among primary causes of RCC. We aimed to report a case of RCC in long-term follow-up patient previously operated for paravertebral neuroblastoma.

Case: A-12-year-old female patient, who at 8 months old presented with decreasing lower extremity movements and urinating difficulties. Upon examination a “mass” approximately 8x2x1.7cm in dimension, extending to the spinal cord in the left paravertebral area between T10-L4 vertebrae consistent with neuroblastoma, was found.

The intra-abdominal component of the mass was excised first followed by extradural excision with L5-T10 laminotomy. The histopathology of the tumor was reported as “neuroblastoma”. The patient classified as being in the unfavorable histology-moderate risk group was given a regimen of A9(Vincristine, Dacarbazine, Ifosfamide) and A8(Cyclophosphamide, Etoposide, Cisplatin, Mesna) protocols.

Post chemotherapy, the patient was followed-up with urinary system USG periodically due to neurogenic bladder. Though the patient's approximately 10-year follow-up was without incident and no mass was observed in the abdomen on the ultrasound three months ago, a solid mass was detected in the lower pole of the right kidney in the latest ultrasound.

On MRI, a 20x22mm solid mass lesion with smooth contours and cystic components, radiologically compatible with RCC was detected in the lower anterior pole of the right kidney, thus an open partial nephrectomy was performed. Histopathological findings were consistent with “eosinophilic solid and cystic renal cell carcinoma”. The tumor-parenchyma was 0.1 cm away from the surgical margin.

Conclusion: It’s been reported in the literature that RCC may rarely occur as a secondary malignant neoplasm after neuroblastoma treatment, making our case very rare.

Nöroblastom Tedavisi Alan Hastada Postoperatif Uzun Dönem Takipte Renal Hücreli Karsinom, Olgu Sunumu

Giriş: Nöroblastom sonrası renal hücreli karsinom (RCC) görülmesinin, patofizyolojisi henüz net olarak anlaşılmasa da, literatürde benzer vaka sunumları mevcuttur. Genetik mutasyonlar, nöroblastom tedavisi için uygulanan kemoterapi ve radyoterapi, RCC gelişiminde ön planda suçlanan nedenlerdendir. Bu çalışmada, daha önce paravertebral nöroblastom nedeniyle opere edilen; uzun dönem takipte RCC gelişen bir olgu sunulması amaçlandı.

Olgu: 12 yaşında kız hasta. 8 aylıkken ailesi alt ekstremite hareketlerinin azaldığını ve idrar yaparken zorlandığını fark etmesi nedeniyle tetkik edilirken, T10-L4 vertebralar arasında, sol paravertebral alanda medulla spinalise uzanımı olan, boyutları yaklaşık 8x2x1,7 cm olan nöroblastom ile uyumlu kite” saptanmış.

Kitlenin önce batın içi komponenti eksize edilmiş, sonrasında L5-T10 laminotomi ile ektradural tümör eksizyonu uygulanmış. Total olarak eksize edilen tümörün histopatolojisi “nöroblastom” olarak raporlanmış. Hasta kötü histoloji-orta risk grubunda olarak değerlendirilerek tedavi programına alınarak A9(vinkristin, dakarbazin, ifosfamid) ve A8(siklofosfamid, etoposid, sisplatin, mesna) protokolleri ile tedavi uygulanmış.

Kemoterapisi tamamlanan hasta, sonrasında nörojen mesane nedeniyle aralıklı üriner sistem USG ile izlenmekteydi. Yaklaşık on yıllık izlemde patoloji saptanmayan hastada, üç ay önceki ultrasonda batın içerisinde kitle izlenmezken, son ultrasonda sağ böbrekte alt polde solid kitle saptanması üzerine başvurdu.

MRG’de sağ böbrek alt polde anteriorda 20x22mm boyutta düzgün kontürlü, kistik komponentleri olan, radyolojik olarak RCC ile uyumlu solid kitlesel lezyon izlenmesi üzerine açık parsiyel nefrektomi uygulandı. Histopatolojik ve immunohistokimyasal incelemede "eozinofilik solid ve kistik renal hücreli karsinom" tanısı aldı. Tümör parankim cerrahi sınırına 0,1 cm uzaklıktaydı.

Sonuç: RCC’nin, nöroblastom tedavisinden sonra nadiren ikincil bir malign neoplazm olarak ortaya çıkabildiği literatürde bildirilmiştir. Bu açıdan olgumuz nadir görülen olgulardandır.
