Evaluation of Pelvic Floor Muscle Functions in Children with Primary Nocturnal Enuresis and the Effect of Combined Pelvic Floor Rehabilitation

Aim: The etiology of Primary Nocturnal Enuresis (PNE) is not known exactly. Bladder control during sleep is achieved through inhibition by activating the striated and smooth pelvic floor muscles (PFM) while inhibiting the detrusor muscle. This neural reflex arc is thought damaged in PNE leading to impaired inhibition of detrusor muscle and lack of activation of PFM. Therefore, the aim of the study is to investigate PFM function and the effect of combined PFM rehabilitation (PFMR) in children with PNE.

Methods: 40 children (11 NME, 29 NMNE) patients diagnosed with PNE as a result of their medical history were included in this study. PFM functions of the children were evaluated with external palpation over the external anal sphincter by the physiotherapist before and after the treatment. Combined PFMR sessions lasting 75 minutes, including urotherapy, alarm theraphy, diaphragm breathing and core exercises, myofascial techniques, and biofeedback training interventions were applied to the patients once a week. The sessions were continued until the patient's symptoms disappeared. DVSS score and enuresis episodes were evaluated before and after treatment.

Results: The mean age of patient was 8 years old. The PFM function of 26 (65 %) patients with PNE were underactive, respectively 6 (15 %) functional, 1 (2,5 %) non-functional and 7 (17,5%) overactive. After the treatment, 38 (95% ) patients were evaluated and 2(5%) patient as underactive. The avarage number of PFMR session was 8,35.

Conclusion: In this study, PFMR is seen as an effective method in the treatment of PNE. Evaluation of PFM function by the physiotherapist plays a role in increasing the effectiveness of the treatment.

Primer Noktürnal Enürezisi Olan Çocuklarda Pelvik Taban Kas Fonksiyonlarının Değerlendirilmesi ve Kombine Pelvik Taban Kas Rehabilitasyonunun Etkinliği

Amaç : Primer Noktürnal Enürezis'in (PNE) etiyolojisi tam olarak bilinmemektedir. Uyku sırasında mesane kontrolü, detrüsör kasını inhibe eden aynı zamanda pelvik taban kaslarını (PTK) (çizgili ve düz kaslar) aktive eden bir mekanizma ile sağlanır. Bu nöral refleks arkının PNE'de hasar görerek detrüsör kasının inhibisyonunun bozulmasına ve PTK'nın aktivasyon eksikliğine yol açtığı düşünülmektedir. Bu nedenle çalışmamızın amacı, PNE'li çocuklarda PTK fonksiyonunu ve kombine PTK rehabilitasyonunun (PTKR) etkisini araştırmaktır.

Yöntem: Bu çalışmaya tıbbi öykü sonucunda PNE tanısı almış 40 çocuk (11 NME, 29 NMNE) hasta dahil edildi. Çocukların PTK fonksiyonları, tedavi öncesi ve sonrası fizyoterapist tarafından eksternal anal sfinkter üzerinden eksternal palpasyon ile değerlendirildi. Hastalara haftada bir seans, 75 dakika kombine PTKR seansları uygulandı. Kombine PTKR içerisine üroterapi, alarm tedavisi, diyafram solunum ve kor egzersizleri, miyofasyal teknikler ve biofeedback eşlikli PTK egzersizleri dahil edildi. Seanslara hastaların semptomları kaybolana kadar devam edildi. Tedavi öncesi ve sonrası DİİSS skoru ve enürezis epizodları değerlendirildi.

Bulgular: Hastaların ortalama yaşı 8 idi. PNE’li yirmi altı (65%) çocuğun PTK fonksiyonu az aktif, altı (15 %) çocuğun fonksiyonel, bir (2,5 %) çocuğun non-fonksiyonel ve yedi (17,5%) çocuğun aşırı aktif olarak değerlendirildi. Tedavi sonrası ise otuz sekiz (95%) fonksiyonel olarak değerlendirilirken iki (5%) çocuk az aktif olarak değerlendirildi. Uygulanan PTKR seansının ortalama sayısı 8,35 idi.

Sonuç: Bu çalışmamızda PNE tedavisinde PTKR etkili bir yöntem olarak görülmektedir. Fizyoterapist tarafından PTK fonksiyonunun değerlendirilmesi tedavinin etkinliğinin artmasında rol oynamaktadır.
