Ureterocalycostomy For Relapse Ureteropelvic Junction Obstruction; A Salvage Procedure

Ureterocalicostomy is a salvage procedure in certain conditions. Although it is performed by open technique traditionally, it is increasingly being performed by laparoscopic and robotic approach. Here we present our ureterocalicostomy experience performed for detoriating hydronephrosis and stasis on scintigraphy, in a 10 years old boy with a history of 2 previous open pyeloplasty for UPJO.

RGP performed intraoperatively revealed images suggesting narrow proximal ureter and relapse UP junction obstruction, and the procedure was proceeded with diagnostic laparoscopy. Due to fibrosis secondary to previous surgery, dissection at the level of proximal ureter was challenging and UPJ could not be exposed. Ureter is ligated as high as possible and lower pole calyx is exposed after amputation of the renal parenchyma. Ureter is spatulated and posterior wall of the uretero-calycostomy anastomosis is performed by 5/0 vicryl in a running fashion. After completion of the posterior wall suturing, 2 ureteral double J stents were introduced (4.7Fr/20 cm). Then anterior wall anastomosis is completed with 5/0 vicryl in a running fashion. The patient was discharged on postoperative 3rd day and the double J stents were removed 1 month after the surgery. US performed 3 and 6 months the surgery revealed improved hydronephrosis and reduced kidney dimensions, andMAG3 scintigraphy performed 6 months after the operation reveals improved drainage.

Ureterocalycostomy procedure is a salvage procedure in selected indications. It provides all of the advantages of minimally invasive surgery when performed laparoscopically.

Nüks Üreteropelvik Bileşke Darlığında Üreterokalikostomi; Kurtarıcı Bir Cerrahi İşlem

Üreterokalikostomi, belirli durumlarda kurtarıcı bir cerrahi işlem olarak uygulanabilecek bir yöntemdir. Klasik olarak açık cerrahi ile uygulanan bu yöntem laparoskopik yada robotik olarak da uygulanabilir. Daha önce 2 kez açık pyeloplasti ameliyatı uygulanmış olan 10 yaşında bir hastada renal hidronefrozda artış ve sintigrafik olarak stazın kötüleşmesi nedeniyle laparoskopik üreterokalikostomi uyguladığımız bir hastayı sunmak istedik.

Intraoperatif yapılan RGP’de proksimal üreter darlığı ve nüks UP darlık düşünülen hastaya laparoskopik eksplorayon yapıldı. Cerrahi eksplorasyon sırasında proksimal üreterde fibrozis nedeniyle UP bileşkeye ulaşılamadı. Proksimal üreter bağlanarak kesildi, böbrek alt pol parenkimi ampute edilerek alt pol kaliksine ulaşıldı. Üreter spatule edilerek 5/0 vicryl ile üreterokalikostomi anastomozunun arka duvarı devamlı teknik ile tamamlandı. Arka duvar anastomozu tamamlandıktan sonra 2 adet double j stent (4.7F/20 cm) yerleştirildi. Daha sonra ön duvar anastomuzu da 5/0 vicryl ile tamamlandı. Postop 3. günde taburcu edilen hastanın stentleri postop 1. ayda çekildi. Postop 3 ve6. Ayında yapılan US’de sağ böbrek boyutları ve hidronefrozda azalma, 6. Ayda yapılan MAG3 sintigrafisinde drenajda düzelme olduğu görüldü.

Üreterokalikostomi prosedürü uygun endikasyonda kurtarıcı bir yöntemdir. Laparoskopik uygulandığında da, minimal invaziv cerrahinin tüm avantajlarını sağlar.
