Our Negative Pressure Wound Therapy Experiences In Wound Treatment Management

Aim: Various wound dressings and mechanical treatment methods such as negative pressure wound therapy (NPWT) are used in the current treatment of adult wound care. In new studies, it is recommended for the treatment of chronic wounds in the pediatric patient group. In NPWT treatment; a sterile sponge filling the wound area is placed and covered with an impermeable transparent cover and connected to the vacuum device with the help of a vacuum tube. The device applies intermittent negative pressure (50-150 mmHg) to the wound site for 72 hours and ensures the formation of granulation tissue. In this study, we wanted to share our NPWT experiences in our own clinic.

Materials and Methods: Patients who used NPWT for chronic wound development between January 2019 and June 2022 were retrospectively scanned. Wound type, NPWT cycle time, presence of wound infection, primary suture or secondary healing status of the wound after NPWT were analyzed retrospectively.

Results: 29 patients who underwent NPWT in the treatment of chronic wounds between January 2019 and June 2022 were retrospectively analyzed. The mean age of the patients was 8.9 years (1 month - 17 years). NPWT on the fascia was applied to 20 patients for open wounds developed after abdominal surgery, 2 patients for wound care after gluteal abscess drainage and 1 patient for burn wounds developed after vascular access extravasation. Special NPWT treatment was applied under the fascia in 3 patients due to deep open wound developed after abdominal surgery, 2 patients due to decubitus ulcer and 1 patient after gastroschisis evisceration. Wound infection was present in 19 patients before the NPWT application and the wound cultures of these patients were clean after the application. Patients received an average of 4.9 cycles (2-16 cycles). After NPWT application, primary suturing was applied to 14 patients and secondary healing was seen in 12 patients.

Conclusion: NPWT treatment facilitates the wound healing process in chronic and acute wounds by reducing morbidity and hospital stay. By reducing the number of dressings; it is beneficial in preventing the psychological trauma caused by dressings in the pediatric patient group.

Yara Yeri Tedavi Yönetiminde Negatif Basınçlı Yara Yeri Terapisi Deneyimlerimiz

Amaç: Erişkin yara bakımı güncel tedavisinde çeşitli yara örtüleri ve negatif basınçlı yara terapisi (NPWT) gibi mekanik tedavi yöntemleri kullanılmaktadır. Yeni çalışmalarda çocuk hasta grubunda da kronik yara tedavisi için önerilmektedir. NPWT tedavisinde; yara yerini dolduran steril bir sünger yerleştirilir ve üzeri sızdırmaz şeffaf bir örtüyle kapatılıp bir vakum tüpü yardımıyla vakum cihazına bağlanır. Cihaz 72 saat boyunca yara yerine aralıklı negatif basınç ( 50-150 mmHg ) uygular ve granulasyon dokusunun oluşumunu sağlar. Bu çalışmamızda kendi kliniğimizdeki NPWT deneyimlerimizi paylaşmak istedik.

Yöntem: Ocak 2019- Haziran 2022 yılları arasında kronik yara gelişmesi nedeniyle NPWT kullanılan hastalar retrospektif tarandı. Hastaların yara tipi, NPWT döngü süresi, yara yeri enfeksiyon varlığı, NPWT sonrası yaranın primer sütürasyon ya da sekonder iyileşme durumu retrospektif olarak incelendi.

Bulgular: Ocak 2019- Haziran 2022 yılları arasında kronik yara tedavisinde NPWT uygulanan 29 hasta retrospektif incelendi. Hastaların yaş ortalaması 8.9 yıl ( 1 ay - 17 yaş ) idi. 20 hastaya batın cerrahisi sonrası gelişen açık yara nedeniyle, 2 hastaya gluteal abse drenajı sonrası gelişen yara bakımı için, 1 hastaya damar yolu ekstravazasyonu sonrası gelişen yanık yarası nedeniyle fasya üstü NPWT tedavisi uygulandı. 3 hastaya batın cerrahisi sonrası gelişen derin açık yara nedeniyle, 2 hastaya dekübit ülseri nedeniyle ve 1 hastaya gastroşizis evisserasyonu nedeniyle fasya altına özel NPWT tedavisi uygulandı. NPWT uygulaması öncesi 19 hastada yara yeri enfeksiyonu mevcut olup bu hastaların uygulama sonrası yara kültürleri temiz sonuçlandı. Hastalara ortalama 4.9 döngü ( 2 - 16 döngü ) uygulandı. NPWT uygulaması sonrası 14 hastaya primer sütürasyon uygulandı, 12 hastada ise sekonder iyileşme görüldü. Tedavi sırasında NPWT dışı nedenlerle 2 hasta ex oldu, 1 hasta ise serbest flep ihtiyacı nedeni ile başka bir kliniğe sevk edildi.

Sonuç: NPWT tedavisi, morbidite ve hastanede kalış süresini azaltarak kronik ve akut yaralarda yara iyileşme sürecini kolaylaştırır. Pansuman sayısını azaltarak; çocuk hasta grubunda pansumanların yarattığı psikolojik travmanın önlenmesinde fayda sağlar.
