A RARE CASE: RECTAL ATRESIA

Rectal atresia is a rare disease that accounts for 1-2% of all anorectal anomalies. Concomitant anomalies are rare. Often the proximal and distal ends are split and the gap is less than 2 cm (Type-2). Anal opening is normal. In the neonatal period, it may present with distal intestinal obstruction symptoms, and delayed cases may present with perforation and sepsis findings. Various corrective techniques have been described.

A 7-day-old male patient was hospitalized with referral from an external center because of inability to remove meconium, biliary vomiting and abdominal distension. Anal opening was normal. The rectal tube did not go beyond 4 cm. There were large air-fluid levels on the direct radiograph of the case. (Picture 1)

No perforation was found in the patient who underwent emergency abdominal exploration. All intestines were highly dilated and distended. It was observed that the catheter guided through the incision made in the sigmoid colon did not reach the distal of the peritoneal reflection. The procedure was terminated by creating a transverse loop stoma. The patient was discharged uneventfully. Anus distance of the atretic segment was 4 cm and there was no gap with fluoroscopy and rectoscopy.

Transanal end-to-end rectal anastomosis (TERA) (Figure 3) was applied at the 9th month of post-operation, and the patient was discharged without any postoperative problem. In the follow-up examination under general anesthesia, mild stenosis was detected in the anastomosis line, and dilatation was performed with Hegar dilators. In the distal colon X-ray of the case, which was easily dilated with no. 16 and rectal washings were performed, it was seen that the contrast agent passed into the rectum without any problems. An operation was planned for stoma closure.

NADİR GÖRÜLEN BİR OLGU: REKTAL ATREZİ

Rektal atrezi tüm anorektal anomalilerin %1-2’sini oluşturan nadir bir hastalıktır. Eşlik eden anomaliler nadirdir. Sıklıkla proksimal ve distal uç ayrıktır ve gap 2cm’den azdır(Tip-2). Anal açıklık normaldir. Yenidoğan döneminde distal intestinal obstrüksiyon semptomlarıyla, gecikmiş olgular ise perforasyon ve sepsis bulgularıyla başvurabilir. Çeşitli düzeltici teknikler tanımlanmıştır.

7 günlük erkek hasta, mekonyum çıkaramama safralı kusma ve batın distansiyonu nedeniyle dış merkezden sevk ile yatırıldı. Anal açıklığı normaldi. Rektal tüp 4cm’den ileri gitmeyen olgunun direkt grafisinde geniş hava-sıvı seviyeleri mevcuttu. (Resim 1)

Acil batın eksplorasyonu yapılan olguda perforasyon saptanmadı. Tüm barsaklar ileri derecede dilate ve distandüydü. Sigmoid kolondan yapılan insizyondan yönlendirilen kateterin peritoneal refleksiyonun distaline ulaşmadığı görüldü. Transvers loop stoma oluşturularak işlem sonlandırıldı. Sorunsuz taburcu edilen olgunun skopi ve rektoskopi ile atretik segmentin anüs uzaklığının 4cm olduğu ve bir gap olmadığı görüldü.

PO 9. ayında transanal end-to-end rektal anostomoz (TERA) (Resim 3) uygulanan olgu postoperatif sorunsuz taburcu edildi. İzleminde yapılan genel anestezi altındaki muayenesinde anastomoz hattında ılımlı darlık saptanan olguya hegar bujileri ile dilatasyon uygulandı. 16 no ile rahat dilate edilen, distal ve rektal yıkamaları yapılan olgunun çekilen distal kolon grafisinde (Resim 4) kontrast maddenin rektuma sorunsuz geçtiği görüldü. Stoma kapatılması için operasyon planlandı.
