Aim: Duodenal duplication cysts (DDC) are rare congenital anomalies of gastrointestinal tract and periampullary localization with biliary and pancreatic duct anomalies remains a surgical challenge. Endoscopic treatment of periampullary DDC (PDDC) communicating with pancreaticobilliary duct in an 18-year-old girl is presented to discuss the endoscopic treatment option in children.
Case report: An 18-months-old girl complaining about abdominal pain and vomiting at 10-months of age underwent abdominal US and revealed a 1.8 x 2 cm cystic mass adjacent to second part of the duodenum. The amylase and lipase levels were increased while the baby was symptomatic. Magnetic resonance cholangiopancreaticography (MRCP) showed thick cyst wall measuring 1.5 x 2 cm at the second part of the duodenum consistent with DDC that has a suspicion of communicating with common bile duct. Upper GI endoscopy confirmed a bulging cyst into the lumen of duodenum. The puncture and injection of the cyst with contrast material confirm the commination of the duplication cyst with the common bile duct. The unroofing of the cyst was performed with endoscopic cautery. The biopsy obtained from the cystic mucosa revealed normal intestinal histology. Oral feeding was initiated six hours after the endoscopy and the patient was discharged next day. She has been following up for the last 8 months uneventfully.
Conclusion: Endoscopic treatment of PDDC with various anatomical variants can be considered as an alternative to surgical excision in children. Long term follow-up is recommended for the endoscopically treated patients, because of potential risk of malignancy.

Amaç: Duodenal duplikasyon kistleri (DDK) gastrointestinal sistemin (GIS) nadir doğumsal anomalilelerinden olup safra yolları ve pankreatik kanala açılan periampuller DDK (PDDK) cerrahi zorluklara neden olmaktadır. 18 aylık kız olgu pankreatikobiliyer kanalla ilişkili PDDK’lerinin endoskopik tedavisi ve sonuçları tartışılmak üzere sunulmuştur.
Olgu sunumu: 18 aylık kız hasta 10 aylıkken karın ağrısı ve kusma şikayeti ile başvurduğu sırada yapılan ultrasonografide duodenumun ikinci kısmına komşu, 1.8 x 2 cm boyutunda bir kist saptanmıştır. Semptomlarının olduğu dönemde bakılan serum amilaz ve lipaz düzeyleri artmış olarak saptanmıştır. Manyetik rezonans kolanjiopankreatikografide, 1.5 x 2 cm boyutunda kalın duvarlı, ortak safra kanalı ile ilişkili olduğu düşünülen PDDK ile uyumu lezyon görülmüştür. Yapılan üst GIS endoskopisinde kistin duodenum lümenini doldurduğu izlenmiştir. Kiste ponkisyon yapıldıktan sonra kontrast madde ile yapılan görüntülemede kistin ortak safa kanalı ile ilişkisi olduğu gösterilmiştir. Endoskopik koter yardımı ile kiste unroofing yapılmıştır. Kist duvarı ve mukozasından yapılan biyopsiler normal intestinal histoloji ile uyumlu saptanmıştır. Endoskopiden 6 saat sonra oral başlanan hasta ertesi gün taburcu edilmiş ve son 8 aydır sorunsuz takip edilmektedir.
Sonuç: Anatomik varyasyon gösteren PDDK’lerde endoskopik tedavi yönetmeleri cerrahi eksizyona alternatif olabilir. Malignite riski nedeni ile endoskopik tedavi yapılan PDDK’lerde uzun dönem takip gerekmektedir.
