Safe appendiceal stump closure with LigaSure™ in laparoscopic appendectomy.

Background: Laparoscopic appendectomy is being performed with increasing frequency in the treatment of acute appendicitis in children. This study aimed to evaluate the efficacy and safety of the LigaSure™ Vessel Sealing System (Valleylab, Boulder, CO) in the closure of the appendix stump during laparoscopic appendectomy.

Patients and Methods: Laparoscopic appendectomy was performed using only the LigaSure system in 21 patients diagnosed with acute appendicitis. The appendicular stump closure was closed with ligasure alone, without the use of endoloops, endoclips, or endostaplers.

Results: Mean operative time was 80.8 ± 24.5 minutes (50-110 minutes). The mean length of postoperative hospital stay was 1.2 ± 0.4 days (range, 1-2 days). There was no evidence of a complication associated with inadequate stump closure with the use of the LigaSure system.

Conclusion: Using only the ligature system for appendiceal stump closure is a safe and effective procedure. From the separation of the mesoappendix to stump closure, completion of a laparoscopic appendectomy using only the LigaSure system safely simplifies the procedure.

Laparoskopik apendektomide LigaSure™ ile güvenli apendiks güdüğü kapatma.

Amaç: Çocuklarda akut apandisit tedavisinde laparoskopik apendektomi giderek artan sıklıkta yapılmaktadır. Bu çalışma, laparoskopik apendektomi sırasında apendiks güdüğünün kapatılmasında LigaSure™ Damar Sızdırmazlık Sisteminin (Valleylab, Boulder, CO) etkinliğini ve güvenliğini değerlendirmeyi amaçlamıştır.

Hastalar ve Yöntemler: Akut apandisit tanısı alan 21 hastaya sadece LigaSure sistemi kullanılarak laparoskopik apendektomi yapıldı. Apendiküler güdük kapatma endoloop, endoklip veya endostapler kullanılmadan sadece ligasure ile kapatıldı.

Bulgular: Ortalama ameliyat süresi 80,8 ± 24.5 dakika (50-110 dakika) idi. Ameliyat sonrası hastanede kalış süresi ortalama 1.2 ± 0.4 gün (aralık, 1-2 gün) idi. LigaSure sisteminin kullanımıyla yetersiz güdüğün kapatılmasıyla ilişkili herhangi bir komplikasyon gelişmedi.

Sonuç: Apendiks güdüğü kapatılmasında sadece ligatür sisteminin kullanılması güvenli ve etkili bir işlemdir. Mezoapendiksin ayrılmasından güdük kapatılmasına kadar, sadece LigaSure sistemi kullanılarak laparoskopik apendektominin tamamlanması, prosedürü güvenli bir şekilde basitleştirmektedir.
