Management of Primary Spontaneous Pneumothorax in Children: Current Practices among Turkish Pediatric Surgeons

Introduction: The correct management of primary spontaneous pneumothorax (PSP) in children is controversial. The aim of this study was to describe current practices among Turkish Pediatric Surgeons.

Methods: An online survey comprising 19 questions was sent out Turkish Association of Pediatric Surgeons (TAPS) to all members. Descriptive analysis was performed on the answers to each survey question, with reporting of frequencies and percentages.

Results: The rate of utilization computed tomography (CT) in the initial admission was 27.4%, and the rate of use in cases requiring surgical treatment was 78.4%. Eighty-one percent of surgeons reported that the size of the pneumothorax influences management. For a first episode of PSP, 75.7 % of surgeons opt for chest tube drainage, 14.9% for only oxygen administration, 4.1% for needle aspiration and none of the participants preferred video-assisted thoracoscopic surgery (VATS). For patients with a persistent air leak after chest tube placement, there was wide variation in duration of observation before performing surgery. VATS was the most preferred surgical intervention (75.3%) and the most preferred surgical method was stapled bullectomy (43.8%).

Conclusions: There are practice variations in the management of pediatric PSP among TAPS members. However, the majority of members seem to agree on the use of CT, initial treatment and surgical interventions. Long-term prospective data are needed to better inform guidelines and standardize practice in children.

Çocuklarda Primer Spontan Pnömotoraks Yönetimi: Türk Çocuk Cerrahlarında Güncel Uygulamalar

Giriş: Çocuklarda primer spontan pnömotoraksın (PSP) yönetimi tartışmalıdır. Bu çalışmanın amacı Türk Çocuk Cerrahları arasındaki güncel uygulamaları belirlemektir.

Yöntemler: Türkiye Çocuk Cerrahisi Derneği'nin (TÇDD) tüm üyelerine 19 sorudan oluşan çevrimiçi bir anket gönderildi. Her anket sorusuna verilen cevaplar üzerinde, frekans ve yüzdelerin raporlanmasıyla analiz yapıldı.

Bulgular: İlk başvuruda bilgisayarlı tomografi (BT) kullanım oranı %27,4, cerrahi tedavi gerektiren olgularda kullanım oranı ise %78,4 idi. Cerrahların yüzde seksen biri, pnömotoraksın boyutunun yönetimi etkilediğini bildirdi. PSP'nin ilk epizodu için cerrahların %75,7'si göğüs tüpü drenajını, %14,9'u sadece oksijen uygulamasını, %4,1'i iğne aspirasyonunu tercih etmekte ve katılımcıların hiçbiri video yardımlı torakoskopik cerrahiyi (VATS) tercih etmemektedir. Göğüs tüpü yerleştirildikten sonra kalıcı hava kaçağı olan hastalar için, ameliyat yapılmadan önce gözlem süresinde büyük farklılıklar vardı. En çok tercih edilen cerrahi girişim VATS (%75.3), en çok tercih edilen cerrahi yöntem ise stapler büllektomi (%43.8) idi.

Sonuçlar: Türkiye Çocuk Cerrahisi Derneği üyeleri arasında pediatrik PSP yönetiminde uygulama farklılıkları vardır. Bununla birlikte, üyelerin çoğunluğu BT kullanımı, başlangıç ​​tedavisi ve cerrahi müdahaleler konusunda hemfikir görünmektedir. Kılavuzları daha iyi bilgilendirmek ve çocuklarda uygulamayı standart hale getirmek için uzun vadeli ileriye dönük verilere ihtiyaç vardır.
