Surgical therapy in a patient with congenital H-type rectovaginal fistula

Introduction: H-type rectovaginal fistula (H-RVF) associated with a normally developed anus is a rare variant of anorectal malformation (ARM). Surgical intervention techniques vary.

Case: A 2,5-month-old girl weighing 4,9 kg, whose first presentation was perineal tragus, was brought with the complaint of stool discharge from vagina on the second day after tragus excision. In the examination, a fistula located anterior to the hymen and extending to the rectum was detected. Anorectal examination was normal. First a stoma was performed. At 8 months old, the definitive surgery was performed. In the operation, the fistula tract was catheterized in the supine position. After suspending the fistula from the vaginal opening with sutures, it was completely dissected up to the level of the anterior rectal wall. Then, a semilunar incision was made anterior to the anus and the fistula tract was reached by dissection proximally, following the rectal wall. The fistula was inverted from the vagina to the rectum and pulled towards the rectum's anterior side. The fistula was ligated and excised at the rectal wall level. The anterior rectal wall was mobilized with a sharp upward dissection and separated by 2 cm from the posterior vaginal wall. The dissected anterior surface of the rectum was excised proximal to the fistula level. The external sphincter muscle was between the vagina and the rectum. Anoplasty was performed. The vaginal opening was primarily repaired. No recurrence was observed in the examination performed 1 month after the operation and the stoma closure was completed. The postoperative third month follow-up is uneventful.

Conclusion:H-RVF occurs in approximately 3% of the entire ARM spectrum. Probably due to its rarity, there is no universally accepted surgical technique. The most common complication after surgery is relaps. In the technique described here, the fistula tract is completely excised, overlapping suture lines are avoided in the opening of vagina and rectal wall. Since no perineal incision is made, the development of anal sphincter dysfunction is not expected. Finally, cosmetic outcomes are satisfactory. The patient's age and weight were considered low for a successful primary surgery, so a stoma was performed but the necessity of opening a stoma before H-RVF surgery is controversial.

Konjenital H-tipi rektovajinal fistüllü bir hastada cerrahi tedavi

Giriş: Normal gelişmiş anüsle birliktelik gösteren H-tipi rektovajinal fistül (H-RVF) nadir bir anorektal malformasyon (ARM) varyantıdır. Cerrahi girişim teknikleri çeşitlilik göstermektedir.

Olgu: İlk başvurusu perineal tragus olan 2,5 aylık ve 4,9 kg ağırlığında kız bebek, tragus eksizyonunu izleyen ikinci günde vajenden gaita gelmesi yakınmasıyla getirildi. Yapılan muayenede himen anteriorunda yer alan ve rektuma uzanan fistül saptandı. Anüs ve rektum muayenesi doğaldı. Hastaya stoma açıldı ve 8 aylıkken definitif cerrahisi yapıldı. Ameliyatta sırt üstü pozisyonda fistül traktusu kateterize edildi. Fistülün vajinal açıklık tarafından askı dikişleriyle asılmasını takiben, rektum ön duvarı seviyesine kadar çepeçevre diseke edildi. Ardından anüs anteriorundan semilunar bir kesi yapıldı ve rektum duvarını takip ederek proksimale doğru diseksiyonla fistül traktusuna ulaşıldı. Fistül vajen tarafından rektum tarafına inverte edildi. Rektum ön duvarı duvarı seviyesinde bağlanarak eksize edildi. Anterior rektum duvarı yukarı doğru keskin diseksiyonla mobilize edildi ve vajenin arka duvarından 2 cm kadar ayrılması sağlandı. Diseke edilen rektum ön yarısı fistül seviyesinin proksimalinde şekilde eksize edildi. Eksternal sfinkter kası, intakt olarak, vajen ile rektum arasında kaldı. Anoplasti tamamlandı. Vajinal açıklık primer onarıldı. Ameliyattan 1 ay sonra yapılan muayenede nüks görülmedi ve stoma kapatıldı. Ameliyat sonrası üçüncü ay takibi sorunsuzdur.

Sonuç: H-RVF tüm ARM spektrumunda yaklaşık %3 oranında görülür. Nadir görülmesi nedeniyle, genel kabul görmüş bir ameliyat tekniği yoktur. Ameliyat sonrası en sık görülen komplikasyon nükstür. Burada tanımlanan teknikte fistül traktusu tamamen çıkartılmış ve ayrıca vajen ile rektum duvarındaki açıklıkta üst üste binen dikiş hattı olmasının önüne geçilmiştir. Perinede kesi yapılmadığından anal sfinkter disfonksiyonu gelişmesi beklenmemektedir. Kozmetik açıdan da son derece tatminkar bir sonuç elde edilmiştir. H-RVF ameliyatı öncesinde stoma açılması gerekliliği tartışmalıdır. Sunulan hastanın yaşının ve tartısının başarılı bir primer ameliyat için düşük olduğu düşünülerek, stoma açılmıştır; ancak, bu konu tartışmaya açıktır.
