Complication feared in circumcision

Objective: In this presentation, we aimed to present the treatment with pentoxifylline and acetyl salicylic acid in a patient who had bleeding and circulatory disorders in the glans three days after being circumcised at home.

Case: In the examination of a 9-year-old male patient, who was evaluated with the complaint of post-circumcision bleeding, the circulation of the glans was impaired and the glans was quite tense. The patient's tightly wrapped bandage was opened. There was no improvement in glans circulation after the dressing was opened. Urinary catheter inserted. Two small incisions were made to the glans penis. The patient was started on a total of 10 mg/kg intravenous pentoxifylline divided into two equal doses per day and 100 mg oral acetyl salicylic acid once daily. It was observed that the glans circulation of the patient improved in the third day. In the evaluation of the patient after 9 days, the circulation of glans returned to normal.

Conclusion: The most common complication after circumcision is bleeding. Although wrapped bandage is effective treatment in post-circumcision bleeding, too tightly bandages can cause circulatory disorders. Circulatory disorder in glans after circumcision is a very rare complication. Frequency of these and similar complications varies according to factors such as circumcision method and monopolar cautery use. In case of circulatory disorders in the penis, the risk of penile necrosis can be prevented with the pentoxifylline and acetyl salicylic acid combination therapy.

Sünnette korkulan komplikasyon

Amaç: Bu sunumda evde sünnet edildikten üç gün sonra kanama ve glansta dolaşım bozukluğu görülen hastada pentoksifilin ve asetil salisilik asit ile tedavisini sunmayı amaçladık.

Olgu: Sünnet sonrası kanama şikayeti ile değerlendirilen 9 yaşındaki erkek hastanın muayenesinde glans dolaşımının bozulduğu ve glansın oldukça gergin olduğu görüldü. Hastanın çok sıkı sarılmış sargısı açıldı. Sargı açıldıktan sonra glans dolaşımında herhangi bir düzelme görülmedi. İdrar sondası takıldı. Glans penise iki küçük insizyon yapıldı. Hastaya günde iki eşit doza bölünerek totalde 10mg/kg intravenöz pentoksifilin ve günde tek doz 100mg oral asetil salisilik asit tedavisi başlandı. Hastanın takibinde üçüncü günde glans dolaşımı düzelmeye başladı, dokuzuncu gün değerlendirmesinde glans dolaşımının normale döndüğü görüldü.

Sonuç: Sünnet sonrası kanama en sık görülen komplikasyondur. Kanamada sargı ile sağlanan kompresyon etkili bir tedavi olmakla birlikte sargının çok sıkı sarılması dolaşım bozukluğuna neden olabilir. Sünnet sonrası glansta dolaşım bozukluğu çok nadir bir komplikasyondur. Uygulanan sünnet yöntemi ve monopolar koter kullanımı sıklıkla suçlanan sebeplerdir. Peniste dolaşım bozukluğu durumunda pentoksifilin ve asetil salisilik asit tedavisi kombinasyonu ile penil nekroza gidiş riski önlenebilir.
