The role of lymphocyte monocytes ratio levels in diagnosis of appendicitis in children

Aims: The purpose of this study was to investigate the diagnostic value of lymphocyte monocytes ratio (LMR) in appendicitis in children.

Methods: Charts of two hundred patients under the age of 18 who were admitted to hospital and gave blood samples due to reasons other than inflammatory conditions or were proved to have mesenteric lymphadenitis and appendicitis (acute or perforated) between 2017-2020 were analyzed retrospectively. Patients were classified into 4 subgroups: Group 1 (healthy children without any inflammation, n=50), Group 2 (mesenteric lymphadenitis, n=50), Group 3 (acute appendicitis, n=50) and Group 4 (perforated appendicitis, n=50).

Results: The median LMR level was significantly lower in both acute and perforated appendicitis group than group 1 and group 2 (p=0,000). LMR levels have strong negative correlation significantly with WBC (r Spearman= - 0.688, p=0,00) and CRP(r Spearman= - 0.684, p=0,00). The cut off value of LMR on admission to predict acute appendicitis was 2,98 with a sensivity of 94% and a specificity of 96% (area under the curve, 0,960, p= 0,000). The cut off value of LMR on admission to predict perforated appendicitis was 2,15 with a sensivity of 96% and a specificity of 72% (area under the curve, 0,865, p= 0,000).

Conclusions: In this study shown that LMR may useful with high sensitivity and specificity in the diagnosis of appendicitis. Thus, negative appendectomies may be decreased.

Çocuklarda apendisit tanısında lenfosit monosit oranı değerinin rolü

Amaç: Bu çalışmanın amacı, apendisit tanılı çocuklarda lenfosit monosit oranının (LMR) tanısal değerini araştırmaktır.

Yöntem: 2017-2020 yılları arasında inflamatuar nedenler dışında hastaneye başvuran ve kan tahlili yapılan sağlıklı çocuklar, mezenterik lenfadenit ve akut veya perfore apendisit tanısı alan 18 yaş altı toplamda 200 hasta retrospektif olarak tarandı. Hastalar 4 alt gruba ayrıldı: Grup 1 (inflamasyon bulgusu olmayan sağlıklı çocuklar, n=50), Grup 2 (mezenterik lenfadenit, n=50), Grup 3 (akut apandisit, n=50) ve Grup 4 (perfore apandisit, n=50).

Bulgular: Median LMR düzeyi hem akut hem de perfore apandisit grubunda grup 1 ve grup 2'ye göre anlamlı olarak daha düşüktü (p=0,000). LMR seviyeleri, WBC (r Spearman= - 0.688, p=0,00) ve CRP (r Spearman= - 0.684, p=0,00) ile anlamlı düzeyde güçlü negatif korelasyona sahipti. Akut apandisit öntanısı ile başvuru sırasındaki LMR'nin eşik değeri 2,98'dir ve sensivitesi %94 ve spesifikliği %96'dır (eğri altındaki alan, 0,960, p= 0,000). Perfore apandisit öntanısı ile başvuru sırasındaki LMR'nin eşik değeri 2,15'tir ve sensivitesi %96 ve spesifikliği %72'dir (eğri altındaki alan, 0,865, p= 0,000).

Sonuçlar: Bu çalışmada LMR değerinin apendisit tanısı koyulmasında yüksek sensitivite ve spesifitesi ile yararlı olabileceği gösterilmiştir. Bu sayede negatif apendektomiler azaltılabilir.
