Infantil mesenchymal tumor in the gastrointestinal tract with NTRK rearrangements by partial ileal obstruction

Introduction

Mesenchymal tumors driven by NTRK fusions are clinically and morphologically heterogeneous group of tumors. We report an infant with a gastrointestinal mesenchymal tumor driven by NTRK fusions who presented with partial ileal obstruction and review of the pertinent literature.

Case

A female baby who was born with normal vaginal delivery at 38 weeks of gestation was admitted with the complaints of poor feding and bilious vomiting on the postnatal 24th day. Her abdominal examination was consistent with abdominal distention following oral feedings. Abdominal X-ray showed dilated loops and multiple air-fluid levels highly suspicious for a partial intestinal obstruction. On the postnatal 44th day the patient was taken to laparotomy because of unsuccessful attempts to reach adequate feeding volumes. During exploration, midgut rotation anomaly and a grossly distended small bowel was noted. The mass was causing partial intestinal obstruction. The mass was resected with 3 cm clear margins and excisional tapering of the proximal intestine was performed with subsequent end-to-end anastomosis. The Ladd’s procedure were performed for the midgut rotation anomaly. The final diagnosis was low-grade spindle cell malign neoplasm with NTRK3- MAGI1 fusion.

Conclusion

As a summary the tumor in our patient was the first mesencimal tumor with NRTK fusion detected with partial ileal obstruction. Although it took a period of 20 days for diagnosis, the tumor was totally resected leading to cure without the need for NTRK inhibitors, at least during our short follow-up period. NTRK fusions are more frequently seen in pediatric tumors than in adult tumors. The identification of these NTRK fusions may facilitate precise cancer diagnosis and thus TRK inhibitor–targeted therapy can be started. It is obvious that search for that mutation in a pediatric gastrointestinal system related mesenchymal tumor may then easen the oncologic therapy leading to prolonged disease free follow-up.

Parsiyel intestinal obstrüksiyon yapan NTRK rearrangementli infantil gastrointestinal mezentimal tümör

Giriş

NTRK füzyonları olan mezenkimal tümörler, klinik ve morfolojik olarak heterojen bir tümör grubudur. Parsiyel ileal obstrüksiyon ile başvuran NTRK füzyonları olan gastrointestinal mezenkimal tümörü olan bir infant ve ilgili literatürün gözden geçirilmesi sunulmaktadır.

Olgu

38. gebelik haftasında normal vajinal yolla dünyaya gelen kız bebek, postnatal 24. günde yetersiz beslenme ve safralı kusma şikayetleri ile başvurdu. Karın muayenesinde oral beslenme sonrası abdominal distansiyon mevcuttu. Karın röntgeni, dilate anslar ve birden fazla hava-sıvı seviyeleri olduğu için şüpheli parsiyel bağırsak obstrüksiyonu gösterdi. Postnatal 44. günde yeterli beslenme volümlerine ulaşılamaması üzerine hasta laparotomiye alındı. Eksplorasyon sırasında orta barsak rotasyon anomalisi ve büyük ölçüde şişkin bir ince barsak saptandı. Kitle kısmi bağırsak tıkanıklığına neden oluyordu. Kitle 3 cm net sınırlarla rezeke edildi ve ardından uç uca anastomoz ile proksimal bağırsağın eksizyonel sivriltilmesi yapıldı. Midgut rotasyon anomalisi için Ladd prosedürü uygulandı. Nihai tanı, NTRK3-MAGI1 füzyonu ile düşük dereceli iğ hücreli malign neoplazmdı.

Sonuç

Özetle hastamızdaki tümör, parsiyel ileal obstrüksiyon ile saptanan ilk NRTK füzyonlu mezenkimal tümördü. Tanı konulması 20 gün gibi bir süre almasına rağmen, en azından bizim kısa takip süremizde NTRK inhibitörlerine ihtiyaç duymadan tümör tamamen rezeke edilerek kür sağlandı. NTRK füzyonları pediatrik tümörlerde erişkin tümörlere göre daha sık görülür. Bu NTRK füzyonlarının tanımlanması, kesin kanser teşhisini kolaylaştırabilir ve böylece TRK inhibitör hedefli tedavi başlatılabilir. Pediatrik gastrointestinal sistem ile ilişkili mezenkimal tümörde bu mutasyonun araştırılmasının daha sonra onkolojik tedaviyi kolaylaştırabileceği ve uzun süreli hastalıksız takibe yol açabileceği açıktır.
