Progressive Repair in a Case with Giant Omphalocele

Introduction: The omphalocele is congenital anterior abdominal wall defect and the abdominal organs are outside the abdomen in a pouch. Although there is no worldwide consensus on its definition, cases where the abdomen defect is larger than 5 cm and the whole or most of the liver in the pouch is called giant omphalocele. This anomaly, is often accompanied by significant additional anomalies such as lung hypoplasia, congenital heart defects and chromosomal disorders. Despite significant improvements in neonatal surgery and intensive care, babies with giant omphalocele still face high mortality rates and high risk of morbidity in survivors. In this article, we present our experiences of gradual repair applied in the treatment of a newborn with isolated giant omphalocele.

Case Presentation: In the examination of the male baby who was born at 3750 gr with 37-week C / S, it was observed that the intestines, liver, stomach and spleen were hernia in a pouch filled with serous fluid from the anterior abdominal wall defect of 12 cm. There was no patent anomaly in the abdominal ultrasound of the patient with patent foramen ovale and ductus opening in ECO. No chromosomal anomaly was found. The patient's omphalocele sac was excised on the 2nd day after delivery. Since the volume of the abdomen is very small, a saline bag is sutured to the edges of the fascia and intra-abdominal organs are taken into the silo. On the 36th day, the silo was removed and the composite mech was sutured to the fascia, and the defect was closed, but the skin was left open because it did not approach. On the 51st day, a silicone extender was stitched around the skin. On day 84, the silicone extender and patch were excised, and the fascia and skin were primarily closed. The patient was discharged with healing 2 weeks after the operation.

Conclusion: Although primary surgical repair is the first preferred method in the treatment of giant omphalocele, it is often not possible as in our patient. In this respect, we achieved a successful result by using a combination of silo, mech and skin extender methods in the gradual repair technique applied in our patient.

Dev Omfaloselli Bir Olguda Aşamalı Onarım

GİRİŞ: Omfalosel doğumsal bir karın duvarı ön duvarı defekti olup, karın içi organlar bir kese içerisinde karnın dışındadır. Tanımı üzerinde dünya genelinde bir fikir birliği olmamasına rağmen, karın defektinin 5 cm’den büyük olduğu ve kese içinde karaciğerin tamamının veya büyük bir kısmının bulunduğu durumlar dev omfalosel olarak adlandırılır. Yaklaşık 10.000 canlı doğumda bir görülen bu anomaliye genellikle akciğer hipoplazisi, konjenital kalp defektleri ve kromozomal bozukluklar gibi önemli ek anomaliler eşlik eder. Günümüzde yeni doğan cerrahisi ve yoğun bakımındaki önemli iyileşmelere rağmen, dev omfaloselli bebekler hala yüksek mortalite oranları ve hayatta kalanlarda ise yüksek morbidite riski ile karşı karşıyadır. Bu yazıda izole dev omfalosel saptanan bir yeni doğanın tedavisinde uygulanan aşamalı onarıma ait deneyimlerimiz sunulmaktadır.

OLGU SUNUMU: 37 haftalık C/S ile 3350 gr doğan erkek bebeğin muayenesinde 12 cm’lik karın ön duvarı defektinden seröz sıvı ile dolu bir kese içerisine barsakların, karaciğerin, midenin ve dalağın tamamının herniye olduğu görüldü. EKO’da patent foramen ovale ve duktus açıklığı görülen hastanın karın ultrasonunda herhangi bir anomali saptanmadı. Herhangi bir kromozomal anomaliye de rastlanılmadı. Doğum sonrası 2. günde opere edilen hastanın omfalosel kesesi eksize edildi. Karın hacmi çok küçük olduğu için fasya kenarlarına serum fizyolojik torbası dikilerek karın içi organlar silo içerisine alındı. 36. gününde silo çıkartılarak fasyaya kompozit yama dikilerek defekt kapatıldı, fakat cilt yanaşmadığı için açık bırakıldı. 51. gününde cilt kenarlarına silikon genişletici dikildi. 84. gününde silikon genişletici ve yama eksize edilerek, fasya ve cilt primer olarak kapatıldı. Ameliyattan 2 hafta sonra hasta şifa ile taburcu edildi.

SONUÇ: Dev omfaloselin tedavisinde primer cerrahi onarım ilk tercih edilen yöntem olsa da çoğunlukla bu hastamızda olduğu gibi mümkün olmamaktadır. Bu açıdan hastamızda uygulanan aşamalı onarım tekniğinde silo, yama ve cilt genişletici gibi yöntemlerinin kombinasyonu kullanarak başarılı bir sonuç elde ettik.
