A Case of Ileal Adenocarcinoma Causing Intestinal Obstruction in a Patient Followed with the Diagnosis of Glioblastoma accompanying ALL and Neurofibromatosis Type 1

IntroductionSmall bowel tumors of childhood constitute 2% of all gastrointestinal tract tumors. Increased incidence of adenocarcinoma has been identified in celiac disease, Chron disease, T-cell lymphoma, Familial Adenomatous Polyposis,Juvenile Polyposis Syndrome and Peutz-Jeghers syndromes.In our study,a patient who was operated for intestinal obstruction and diagnosed with ileal adenocarcinoma on histopathological examination is presented.

CaseA 9-year-old girl was diagnosed with B Cell Lymphoblastic Leukemia and Neurofibromatosis Type 1 in the year of 2015.The patient was started on chemotherapy and the patient was in remission.The patient was diagnosed with right frontal located Glioblastoma Multiforme in 2015 and the patient was operated.In January 2020, a recurrent mass was detected.While the patient was followed up in the intensive care unit after intracranial mass excision, our clinic was consulted due to bile vomiting.The patient was cachectic and had no abdominal distension, tenderness, and defense in his physical examination.No pathological finding was detected on the patient's direct radiography and abdominal ultrasound.Nasogastric catheter was inserted and a small amount of bile fluid came out of the catheter.The patient was thought to have Superior Mesenteric Artery Syndrome and a gastric duodenal passage graph was taken, no pathological finding was detected.During the follow-up of the patient, it was planned to be explored due to the increase in the nasogastric catheter and increased abdominal distention.In the exploration, approximately 70 cm distal of Treitz, there was a ring-shaped circular thickening that narrowed the lumen and no additional abdominal pathology was detected.The lesion was resected by leaving 2 cm normal jejunal tissue proximally and distally, and anastomosis was performed.Histopathological examination of the lesion was reported as adenocarcinoma and no tumor at the surgical margins.In the PET Computerized Tomography examination, there was no F-18 FDG involvement in the abdomen.As a result of genetic evaluation,homozygous variant was detected in the MLH 1 gene.

ResultIn some diseases where the incidence of adenocarcinoma increases,in cases of intestinal obstruction,it should be evaluated that the cause may be a bowel originated tumor.In the treatment of small bowel adenocarcinoma,it is recommended to perform wide bowel resection in order to protect from surgical margin positivity and regional lymph node excision

ALL ve Nörofibromatozis Tip 1’e eşlik eden Glioblastom Tanısı ile Takipli Hastada İntestinal Obstruksiyona Neden Olan Bir İleal Adenokarsinom Olgusu

Giriş

Çocukluk çağının ince bağırsak tümörleri tüm gastrointestinal sistem tümörlerinin %2’sini oluşturmaktadır. Çölyak hastalığı, Chron hastalığı, T-cell lenfoma, Familial Adenomatöz Polipozis, Juvenil Polipozis Sendromu ve Peutz-Jeghers sendromlarında adenokarsinom insidansının arttığı tanımlanmıştır. Çalışmamızda, intestinal obstruksiyon nedeni ile ameliyat edilen ve histopatolojik incelemede ileal adenokarsinom tanısı konulan bir hasta sunulmuştur.

Olgu

9 yaşında kız hastaya, 2015 yılında B Hücreli Lenfoblastik Lösemi ve Nörofibromatozis Tip 1 tanısı konulmuştur. Hastaya kemoterapi başlanmış ve hasta remisyona girmiştir. Hastada 2015 yılında sağ frontal yerleşimli Glioblastom Multiforme tanısı konuldu ve hasta opere edildi. Ocak 2020’de sağ frontoparietal bölgesinde nüks kitle saptandı. İntrakranial kitle eksizyonu sonrası yoğun bakımda takip edilirken, dirençli safralı kusmaları nedeniyle kliniğimize danışıldı. Hasta kaşektik olup, fizik muayenesinde abdominal distansiyonu, hassasiyeti, defansı yoktu. Hastanın ayakta direkt grafisinde ve abdominal ultrasonunda patolojik bulgu saptanmadı. Nazogastrik katater yerleştirildi ve az miktarda safralı geleni oldu. Hastada Süperior Mezenterik Arter sendromu olabileceği düşünüldü ve mide duodenum pasaj grafisi çekildi, patolojik bulgu saptanmadı. Takibinde hastanın distansiyonu belirginleşti ve nazogastrik kataterden gelenlerinin artması üzerine eksplore edilmesi planlandı. Yapılan eksplorasyonda Treitz’ın yaklaşık 70 cm distalinde, lümeni daraltan yüzük şeklinde sirküler kalınlaşma mevcuttu ve ek patoloji saptanmadı. Lezyon proksimal ve distalinde 2 cm normal jejunal doku bırakılarak rezeke edildi ve anastomoz yapıldı. Lezyonun histopatolojik incelemesi, Adenokarsinom ve cerrahi sınırlarda tümör yoktu olarak rapor edildi. Çekilen PET Bilgisayarlı Tomografi incelemesinde batında F-18 FDG tutulumu yoktu. Genetik değerlendirme sonucu hastada MLH 1 geninde homozigot varyant tespit edildi.

Sonuç

Adenokarsinom gelişme İnsidansının arttığı bazı hastalıklarda intestinal obstrüksiyon gelişmesi halinde nedeninin bağırsak kaynaklı tümör olabileceği değerlendirilmelidir. İnce bağırsak adenokarsinomunun tedavisinde bölgesel lenf nodları ile en-bloc kitle rezeksiyonu ve cerrahi sınır pozitifliğinden korunmak için geniş barsak rezeksiyonu ile birlikte önerilmektedir.
