FOLLOW-UP OF ISOLATED ESOPHAGENIC ATRESIS WITHOUT ESOPHAGASTOMY AND RESULTS

Objective: The most difficult management of esophageal atresia cases is long gap isolated esophageal atresia cases. In this study, our cases, which we have successfully applied in our case series: drainage and expansion of the proximal pouch with nosagastric without esophagostomy, and intrathoracic anastomosis with late gastric retraction, are presented. Method : Eleven patients diagnosed with isolated esophageal atresia in our clinic between 2012-2021 were included in the study. Gastrostomy was performed in all cases. None of them underwent esophagostomy. Postoperative gastrostomy feeding was started and the family was taught about proximal pouch drainage with tight fixation of the nasogastric tube (Ng). Pouch growth was followed by weekly check-ups. Ng was advanced as growth in the pouch was observed. Surgery was planned when the patients reached 10 kg. Results: The mean preoperative age of the cases was 9.7 months (8.3-11.6 months). The growth of the pouch followed by Ng was 1.9 cm (1.6-2.3 cm) during the follow-up period of the cases consisting of two girls and 9 boys. At the end of the follow-up, gastric pull-up was performed by peroperative gap measurement in all patients. The median discharge time was 12 days. Antacid and gastric protective (PPI) were started in all discharged patients. No complications were observed in the early postoperative period. In the late period, 2 cases were included in the dilatation program due to stenosis. No mortality was observed. Conclusion: Isolated esophageal atresia cases are classically repaired with early esophagostomy and then extrathoracic anastomosis. However, with the method we follow, patients can be protected from aspiration with a catheter in the pouch without esophagostomy, and they can be treated effectively and safely with gastric pull-up surgery in the late period, thanks to the salivary secretion pressure and the proximal esophagus prolonging with the driving force of ng.

İZOLE ÖZOFAGUS ATREZİSİ OLGULARININ ÖZOFAGASTOMİSİZ TAKİBİ VE SONUÇLAR

Amaç: özofagus atrezisi olgularının en zor yönetileni uzun aralıklı (long gap) izole özofagus atrezili olgulardır. Bu çalışmada kendi olgu serimizde başarıyla uyguladığımız:özofagostomi yapmadan nozagastrik ile proksimal poşun drenajı ve genişletilmesi, geç dönemde midenin toraksa çekilmesiyle intratorasik anostomoz yapılan olgularımız sunuldu.

Yöntem : Kliniğimizde 2012-2021 arası 11 izole ozofagus atrezisi tanılı olgu çalışmaya alındı. Olguların hepsine gastrostomi açıldı. Hiç birine özogafagostomi açılmadı. Ameliyat sonrası gastrostomiden beslenme başlandı ve Nazogastrik sondanın (Ng) sıkı tespiti ile proksimal poş drenajı aileye öğretildi. Haftalık kontrollerle poş büyümesi takip edildi. Poştaki büyüme gözlendikçe Ng ilerletildi. Hastalar 10 kg'a ulaşınca cerrahi planlandı.

Bulgular: Olguların ameliyat öncesi ortalama yaş 9.7aydı(8.3-11.6 aylık). İki kız 9 erkekten oluşan olguların takıp süresinde Ng ile izlenen poş büyümesi 1.9 cm’di (1.6-2.3 cm). İzlem sonu tüm hastalara peroperatif gap ölçümü yapılarak gastrik pull-up yapıldı. Ortanca taburcu süresi 12 gündü. Taburcu olan tüm olgulara antasit ve mide koruyucu (PPI) başlandı. postop erken dönemde komplikasyon görülmedi. Geç dönemde 2 olgu darlık nedeniyle dilatasyon programına alındı. Mortalite gözlenmedi.

Sonuç: İzole özofagus atrezisi olgular klasik olarak erken dönem özofagostomi ve daha sonra ekstratorasik anostomoz ile onarılmaktadır. Fakat izlediğimiz yöntemle hastalar özofagostomi yapmadan poştaki sonda ile aspirasyondan korunabilir, tükrük salgısı basıncı, ve ng nin itici gücüyle uzayan proksimal özofagus sayesinde geç dönemde gastrik pull - up ameliyatı ile etkili ve güvenli şekilde tedavi edilebilir.
