The patient and parent-focused approach in the physiological and pathological breast development in children

Aim: With our clinical approach to neonatal, children and adolescents aged 0-17 years who presented to the pediatric and pediatric surgery outpatient clinic with breast complaints, we planned to analyze the anxiety levels of the patient and parent during the first and last admission.

Patients and Methods: This prospective study was performed between April 2015 and April 2018. The history, physical examination, examination, treatment, and follow-up data of male and female patients aged between 0-17 years of breast complaints were collected and analyzed. Ultrasound (US) was used for imaging when necessary. According to the results of US, hormonal and advanced imaging tests were performed in selected patients, followed by fine needle biopsy in patients with suspected malignancy. Surgery was performed under local or general anesthesia when necessary. Also, parents 'and adolescent patients' attitudes towards the present complaint were compared with the Beck Anxiety Inventory (BAI) at the initial and final follow-up examinations.

Results: Of the 7345 patients admitted to our outpatient clinic during a three-year study, 189 (2.5%) had breast complaints. The mean age of neonatal, prepubertal, and pubertal patients was as follows; 3.73 ± 1.87 (weeks), 5.57 ± 1.67 (years), and 14.25 ± 1.50 (years). Out of 189 patients, 62 (32.8%) were male, and 127 (67.1%) were female. There was no difference in terms of gender in patients presenting with breast complaints (p = 0.79). The highest number of patients (100; 53%) was in the pubertal group. There were 56 (29.6%) patients in the prepubertal group and 33 (17.4%) patients in the neonatal group (Table 1). The findings, diagnosis, treatment, follow-up results and the first examination and last follow-up BAI scores of the pubertal patients and the parents of the three groups according to the complaints of each group were as shown in Table 2a, b.

Conclusion: In the literature; Although breast changes in children are generally benign in character and self-limiting, it has been reported to be of concern to patients and parents. Interestingly, there was no statistically significant difference in terms of breast complaints in children, unlike adults. Our results were consistent with the literature in terms of clinical data. However, as in our study, the extent to which the complaints or illness is of concern to the patient and his family has not yet been investigated according to any scientific scale. In our study, BAI scores decreased at the last follow-up. In addition, although all patients were compared with each other, although the patient was informed in detail, the severity of the score generally differed significantly.Namely, there was a different severity of anxiety for each group.

Çocuklarda fizyolojik ve patolojik meme gelişiminde hasta ve ebeveyn odaklı yaklaşım

Amaç:Çocuk ve çocuk cerrahisi polikliniğine meme şikâyetleriyle başvuran 0-17 yaş arasındaki neonatal, çocuk ve ergenlere klinik yaklaşımımızla birlikte, hasta ve ebeveynin ilk ve son başvurusu sırasındaki kaygılanma düzeylerini analiz etmeyi planladık.

Hastalar ve Yöntem:Prospektif olan bu çalışma Nisan 2015-Nisan 2018 tarihleri arasında yapıldı.Polikliniğine başvuran 0-17 yaş arasındaki erkek ve kız meme şikâyeti olan hastaların; hikâye, fizik muayene, tetkik, tedavi ve takip verileri toplanarak sonuçlar analiz edildi. Gerektiğinde görüntüleme için ultrason (US) kullanıldı. US sonucuna göre, özellikli hastalarda hormonal tetkikler ve ileri görüntüleme tetkikleri yapılması ve arkasından malignite şüphesi olan hastalara ince iğne biyopsisi yapılması planlandı. Cerrahi müdahale gerektiğinde, lokal veya genel anestezi altında yapıldı. Ayrıca, başvuran hastaların ebeveynlerinin ve ergen olan hastaların mevcut şikâyetle ilgili tutumu Beck Anksiyete Ölçeği (BAÖ) ile ilk başvuru ve son takip muayenesinde kıyaslanarak değerlendirildi.

Bulgular:Toplam 7345 hastadan 189 (%2.5)’unda meme şikayeti mevcuttu.Neonatal, prepubertal ve pubertal hastaların yaş ortalamaları sırasıyla;3.73±1.87(hafta), 5.57±1.67(yıl) ve 14.25±1.50(yıl) idi.Toplam 189 hastadan 62(%32.8)’si erkek ve 127(%67.1)’si kız hasta idi. Meme şikayeti ile başvuran hastaların cinsiyet açısından aralarında bir fark yoktu(p = 0.79). En fazla hasta sayısı(100; %53) pubertal grupta idi.Prepubertal grupta 56(%29.6) hasta ve neonatal grupta 33(%17.4) hasta mevcuttu(Tablo 1).Her bir grubun şikâyetine göre bulgu, tanı, tedavi, takip sonuçları ve pubertal hasta ile 3 grubun ebeveynlerinin ilk muayene ve son takip BAÖ skorları Tablo 2a,b’deki gibiydi .

Sonuç:Literatürde; çocuklardaki cinsiyet gelişiminde, meme değişiklikleri genellikle iyi huylu olmasına rağmen hasta ve ebeveynleri endişelendirdiği bildirilmiştir.İlginç olarak erişkinden farklı olarak çocuklarda meme şikâyetleri açısından istitatistiksel olarak fark yoktu.Bizim sonuçlarımız klinik veriler açısından literatürle uyumluydu.Fakat, şikayetlerin veya hastalığın hasta ve ailesi için ne kadar endişe verici olduğu henüz herhangi bir bilimsel ölçeğe göre araştırılmamıştır.Çalışmamızda ilk başvuru BAÖ skorları son takipte düştü.Fakat, ayrıntılı hasta bilgilendirilmesine rağmen, tüm gruplar birbirleriyle karşılaştırıldığında, skor şiddetleri genellikle anlamlı şekilde farklılık göstermekteydi.Yani her grup için farklı kaygılanma şiddeti vardı.
