Pre-op analysis of portoenterostomy prognosis in patients with biliary atresia

Objective: Portoenterostomy is the first step surgery to provide bile flow in patients with biliary atresia. For patients with biliary atresia, the effectiveness of postoperative treatment is very important due to transplantation posibility. We aim to predict long-term survival and prognosis of portoenterostomy patients by analyzing time of admission and operation, as well as pre-op observations such as blood values, imaging and biopsy examination.

Methods: In this study, records of 26 patients who underwent portoenterostomy with the diagnosis of biliary atresia between 2007 and 2018 were analyzed retrospectively, where updated information about the prognosis of 17 patients was obtained via followups over the telephone. Gender, birth, admission to our hospital, operation times, pre-op liver function tests (ALT, AST, GGT, ALP, TBL, DBL), APTT, INR levels, viral screening results (hepatotropic viruses); ultrasonography and liver scintigraphy results, surgical notes, pathology reports were examined.

Conclusion: Out of the 17 patients with updated prognostic information, 4 have passed away within 6 months post-operation (2 of which died after transplantion). 6 of the remaining 13 patients underwent transplantation. Cross examination of patients in two groups of good prognosis (those in no need for transplantation) and bad prognosis (those who underwent transplantation or passed away) revealed that the good prognosis group underwent Kasai at an early age and that, contrary to expectations, no correlation was found between pre-op blood values and the type of prognosis.

Biliyer atrezili hastalarda portoenterostomi prognozunun pre-op analizle değerlendirilmesi

Amaç: Portoenterostomi biliyer atrezili hastalarda safra akımı sağlamak için yapılan ilk basamak ameliyattır. Biliyer atrezi hastaları için transplantasyon ihtimali nedeniyle ameliyat sonrası dönem tedavi etkinliği son derece önemlidir. Portoenterostomi yapılan hastaların başvuru ve operasyon vakitleri ile pre-op kan değerleri, görüntüleme ve biyopsi gibi incilemeler üzerinden uzun dönem sağ kalımı ve prognozu öngörmeyi amaçlamaktayız.

Yöntem: Çalışmamızda 2007-2018 arası biliyor atrezi tanılarıyla portoenterostomi yapılmış 26 olgunun kayıtlarına geri dönük olarak ulaşıldı, bunlardan 17 hastanın prognozu hakkında güncel bilgilerine telefon üzerinden ulaşılmıştır. Olguların cinsiyet, doğum, hastanemize başvuru, ameliyat zamanları, pre-op karaciğer fonksiyon testleri (ALT, AST, GGT, ALP, TBL, DBL), APTT, INR düzeyleri, viral tarama sonuçları (hepatotrop virüsler); ultrasonografi ve karaciğer sintigrafisi sonuçları, ameliyat notları, patoloji raporları incelenmiştir.

Sonuç: Ulaşılan 17 hastanın 4’ü post-op 6 ay içinde ex (ex olanların 2’si transplantasyon sonrası ex), kalan 13 hastanın 6’sı transplantasyona gitmiştir. Hastaların pre-op değerlendirilmesi ile iyi prognoz olarak adlandırılan transplantasyona gereksinim duyulmaması ile kötü prognoz olarak adlandırılan transplantasyona gereksinim ya da hayatını kaybeden iki grup karşılıklı incelendiğinde Kasai ile iyi prognozda olan gurubun erken ameliyat edildiği görülmüşken beklenen aksine pre-op kan değerleri ile prognoz arasında ilişki bulunamamıştır.
