A rare cause of cecal perforation: inflammatory bowel disease mimicking hip joint arthritis

Background/Aim: Inflammatory bowel disease (IBD) is a chronic gastrointestinal system disorder that is frequently seen in young adults and adolescents but has an increasing incidence in childhood. Here, we report a case of inflammatory bowel disease who underwent surgery for cecal perforation during follow-up with hip joint arthritis.

Case: A 13-year-old girl was admitted to the orthopedics and pediatric outpatient clinic with complaints of right hip pain and difficulty in walking for last 15 days. Acute phase reactants were found to be increased and intravenous antibiotic therapy was initiated considering septic arthritis of the hip joint. However, fever, vomiting and abdominal pain were added to her complaints, and pediatric surgery consultation was requested because ultrasonography showed the findings consistent with perforated appendicitis. Physical examination revealed tenderness in the right lower quadrant and fullness was palpated. Patient underwent laparotomy and exploration revealed cecal perforation. Pericecal tissues, terminal ileum and appendix were inflammed and edematous. Right hemicolectomy and ileocolic anastomosis were performed. Histopathological examination showed presence of inflammatory bowel disease. The patient was discharged on postoperative day 11. The patient has been cooperatively followed up with pediatric gastroenterology for 6 months without any complaint.

Conclusion: Inflammatory bowel disease rarely leads to an acute onset clinical picture mimicking septic hip joint arthritis. This clinical picture may result in delay in surgical treatment and complications. However, the complications of the appropriate surgical approach can be treated or contributed to the diagnosis.

Çekum perforasyonun nadir bir nedeni: Kalça eklemi artritini taklit eden inflamatuvar bağırsak hastalığı

Önbilgi/Amaç: İnflamatuvar bağırsak hastalığı (İBH), sıklıkla genç erişkin ve adölesanlarda görülen ancak çocukluk çağında insidansı gittikçe artan kronik bir gastrointestinal sistem bozukluğudur. Burada, kalça eklemi artiti bulguları ile başvuran takip sırasında çekum perforasyonu gelişmesi nedeniyle ameliyat edilen bir inflamatuvar bağırsak hastalığı olgusunun sunulması amaçlanmıştır.

Olgu: 13 Yaşındaki kız hasta, 15 gündür olan sağ kalça ağrısı ve sonrasında gelişen yürüme güçlüğü yakınması ile ortopedi ve çocuk hastalıkları polikliniğine başvurmuş. Akut faz reaktanlarında artış saptanmış ve kalça eklemi septik artriti düşünülerek intravenöz antibiyotik tedavisi başlanmış. Ancak yakınmalarına ateş, kusma ve karın ağrısı eklenmesi üzerine ultrasonografi yapılmış ve perfore apandisit ile uyumlu bulgular saptanarak çocuk cerrahisi konsültasyonu istenmiş. Fizik bakıda sağ alt kadranda duyarlılık saptanan ve dolgunluk palpe edilen hastaya laparotomi yapıldı. Eksplorasyonda çekal perforasyon saptandı. Periçekal dokular, terminal ileum ve apendiksin inflame ve ödemli olduğu görüldü. Hastaya sağ hemikolektomi ve ileokolik anastomoz yapıldı. Histopatolojik incelemede inflamatuar bağırsak hastalığı ile uyumlu bulgular saptandı.‎ Postoperatif dönemi olağan seyreden hasta 11 gün sonra taburcu edildi. Olgu, herhangi bir yakınması olmaksızın çocuk gastroenterolojisi ile birlikte 6 aydır izlenmektedir.

Sonuç: İnflamatuvar bağırsak hastalığı, nadiren septik kalça eklemi artritini taklit eden akut başlangıçlı bir klinik tabloya yol açabilir. Bu klinik tablo cerrahi tedavide gecikme ve komplikasyon ile sonuçlanabilir. Ancak, uygun cerrahi yaklaşım komplikasyonları tedavi edilebileceği gibi tanı konulmasına da katkı sağlar.
