Our experiences of nissen fundoplication in children

Aim: Nissen fundoplication is a method of surgical treatment of gastroesophageal reflux disease (GERD). In this study, we aimed to present our experience of Nissen fundoplication in pediatric patients with GERD and / or susceptibility to GERD.

Material and Method: Data of 104 patients who underwent Nissen fundoplication between August 2003-December 2018 has been analyzed retrospectively. Demographic characteristics, symptoms, comorbidities, surgical technique, gastrostomy, complications and follow-up periods were evaluated.

Findings: The mean age of patients undergoing Nissen fundoplication was 4.72 ± 4.94 (2.5 months-216 month). Of the patients, 48 (46.1%) were female and 56 (53.9%) were male. The most common symptoms were vomiting, growth retardation due to malnutrition, and chronic lung disease due to aspiration. When evaluated in terms of additional anomalies; 47 (45.1%) patients had neurological, 27 (25.9%) patients had neurometabolic, 8 (7.7%) patients had muscle disease. 40 (38.4%) patients underwent open Nissen fundoplication, 64 (59.6%) underwent laparoscopic Nissen fundoplication. 82 (78.8%) patients underwent gastrostomy in addition to Nissen fundoplication. Nissen fundoplication with gastrostomy was performed in 24 (23%)patients with neurometabolic disease who required PEG for swallowing dysfunction. Six patients (5.7%) were re-evaluated because of recurrence of GERD or wrap sliding to thorax. Redo Nissen fundoplication was performed in six (5.7%) patients. Heineke-mikulicz phyloroplasty in addition to Nissen fundoplication was performed in four patients who had prolonged gastric emptying time after patients’ examination. Two patients with neurometabolic disease and swallowing dysfunction were first implanted with PEG and Nissen fundoplication was performed when GERD was detected during follow-up.

Results: Nissen fundoplication is an effective and safe method that can be used not only in the treatment of GERD but also in the patients who will undergo gastrostomy due to metabolic disorders and swallowing dysfunction due to neurological diseases with increased incidence of GERD. Gastric emptying time should be evaluated in recurrent cases and pyloroplasty should be added to the treatment in patients who have prolonged gastric emptying time.

Çocuklarda nissen fundoplikasyonu deneyimlerimiz

Amaç: Nissen fundoplikasyon, gastroözefageal reflü hastalığının (GÖRH) cerrahi tedavisinde uygulanan bir yöntemdir. Bu çalışmada GÖRH olan ve/veya GÖRH’e yatkınlığı olan çocuk hastalardaki Nissen fundoplikasyonu deneyimlerimizi sunmayı amaçladık.

Gereç ve Yöntem: Anabilim Dalımızda Ağustos 2003-Aralık 2018 tarihleri arasında Nissen fundoplikasyonu uygulanan 104 hastanın verileri geriye dönük olarak tarandı. Hastaların demografik özellikleri, semptomları, ek hastalıkları, uygulanan cerrahi teknik, gastrostomi uygulanması, komplikasyonlar ve takip süreleri değerlendirildi.

Bulgular: Nissen fundoplikasyonu uygulanan hastaların ortalama yaşı 4,72 ± 4,94 (2,5 ay-216 ay). Hastaların 48’i (%46,1) kız, 56’sı (%53,9) erkekti. En sık görülen semptomlar kusma, beslenememeye bağlı büyüme ve gelişme geriliği, aspirasyon nedeniyle gelişen kronik akciğer hastalığıydı. Ek anomaliler açısından değerlendirildiğinde; 47 (%45,1) hastada nörolojik, 27 (%25,9) hastada nörometabolik, 8 (%7,7) hastada kas hastalığı vardı. 40 (%38,4) hastaya açık Nissen fundoplikasyon uygulanırken, 62 (%59,6) hastaya laparoskopik Nissen fundoplikasyon uygulandı. 82 (%78,8) hastaya Nissen fundoplikasyona ek olarak gastrostomi uygulandı. Nörometabolik hastalığı olan ve yutma disfonksiyonu nedeniyle PEG istenen 24 (%23) hastada gastrostomi ile birlikte Nissen fundoplikasyonu uygulandı. Altı (%5,7) hasta GÖR’ün tekrar etmesi ve/veya wrap’in toraksa yer değiştirmesi nedeniyle tekrar değerlendirildi. Yapılan tetkikleri sonucunda mide boşalması gecikmiş olarak saptanan dört hastaya Nissen fundoplikasyona ek olarak Heineke-Mikulicz piloroplasti yapıldı. Nörometabolik hastalığı ve yutma disfonksiyonu olan iki hastaya ise önce PEG takılıp, takiplerinde GÖRH saptanması üzerine Nissen fundoplikasyonu yapıldı.

Sonuç: Nissen fundoplikasyonu sadece GÖRH tedavisinde değil GÖRH sıklığının arttığı nörolojik hastalıklar ile kas hastalıklarına bağlı yutma disfonksiyonunda ve metabolik hastalıklar nedeniyle gastrostomi yapılacak hastalarda da kullanabilen etkili bir yöntemdir. Nüks eden olgularda mide boşalma zamanı değerlendirilmeli, uzamış olduğu saptanan hastalarda piloroplasti tedaviye eklenmelidir.
