The Effects of Bladder Diverticulum Secondary to Partial Bladder Outlet Obstruction: an Urodynamic Evaluation in Guinea Pigs

ABSTRACT

Aim: The aim of this experimental study was to histopathologically examine the physiology and effects of bladder diverticulum ( BD) secondary to partial bladder outlet obstruction (BOO) in guinea pigs. To our knowledge, this study is the first of its design to investigate this topic.

Method: Guinea pigs (n = 32) weighing 900–1000 g were divided randomly into four groups: the sham group (group 1), BD group (group 2), partial BOO group (group 3), and bladder diverticulum and bladder outlet obstruction group (group 4). All guinea pigs in the four groups underwent urodynamic evaluation preoperatively and at 1 month postoperatively. The bladder capacity, bladder compliance, involuntary detrusor contraction (IDC), filling-phase detrusor pressure (FPdet), and voiding-phase detrusor pressure (VPdet) were recorded. The test subjects’ bladders were removed and examined histopathologically.

Results: No significant difference in VPdet was found among the groups (P = 0.086). The postoperative FPdet was lower in group 1 than in groups 2 and 4 (P < 0.05), and was significantly increased postoperatively relative to the preoperative values in groups 2 and 4 (P = 0.011). Postoperative bladder compliance levels were higher in group 1 than in groups 2 and 4 (P < 0.05). Bladder compliance in groups 2 and 4 was significantly decreased postoperatively compared with the preoperative levels (P = 0.011), with no difference between these groups. IDC was not observed in group 1 or 3, but was observed in 37.5% of subjects in groups 2 and 4; this difference was not significant. On histological examination, no significant difference was found between groups 2 and 4 in the thickness of the diverticulum muscle layer (χ2 = 1.867, P = 0.601) or in the increase in connective tissue in the bladder muscularis mucosa (χ2 = 10.057, P = 0.346).

Conclusion: Secondary BD can alter bladder storage and emptying and over time can reduce the elasticity of the bladder wall. BD can also lead to involuntary contractions. Physiological and histopathological changes in the bladder with BD are more evident than partial BOO.

KOBAYLARDA PARSİYEL MESANE ÇIKIMI OBSTRÜKSİYONUNA SEKONDER GELİŞEN MESANE DİVERTİKÜLÜNÜN İŞEME FİZYOLOJİSİNE ETKİSİ

Amaç: Bu deneysel çalışmanın amacı kobaylarda parsiyel mesane çıkış tıkanıklığından sonra gelişen mesane divertikülünün fizyolojisini ürodinamik olarak ve mesaneyi histopatolojik olarak incelemektir. Bildiğimiz kadarıyla, bu çalışma bu konuyu araştıran ilk tasarımdır.
Yöntem: 900-1000 g ağırlığındaki kobaylar (n = 32) rastgele olarak dört gruba ayrıldı: Sham grubu (grup 1), Mesane divertikülü grubu (Grup 2), kısmi mesane çıkış tıkanıklığı grubu (Grup 3) ve mesane divertikülü ve mesane çıkışı tıkanma grubu (Grup 4). Dört gruptaki tüm kobaylar ameliyat öncesi ve ameliyat sonrası 1 ayda ürodinamik değerlendirme yapıldı. Mesane kapasitesi, mesane uyumu, istemsiz detrusor kasılması (IDC), dolum fazı detrusor basıncı (FPdet) ve boşaltma fazı detrusor basıncı (VPdet) kaydedildi. İşlem sonrası mesaneler çıkarıldı ​​ve histopatolojik olarak incelendi.
Bulgular: VPdet'te gruplar arasında anlamlı fark bulunmadı (P = 0,086). Postoperatif FPdet, grup 1'de grup 2 ve 4'ten daha düşüktü (P <0.05) ve ameliyat sonrası grup 2 ve 4'teki preoperatif değerlere göre anlamlı derecede artmıştı (P = 0.011).
