Intussusception caused by Peutz Jeghers syndrome; Case Report

Introduction and purpose:

Peutz Jeghers Syndrome is an autosomal dominant disorder. Peutz Jeghers Syndrome is characterized by hamartomatous polyps and mucocutaneous pigmentation in the gastrointestinal tract. Although invagination is frequently encountered in children between 5 and 12 months, it is rarely seen in the proximal intestisnal segments. We present a patient with Peutz Jeghers Syndrome had been refered to hospital constantly because of recurrent abdominal pain wherefore she was operated on intussusception.

Case Presentation:

A 6 years old, female patient was hospitalized in pediatric clinic for intermittan, recurrent abdominal pain. Patient who weas evaluated by pediatric surgeon for acute abdominal pain, did not have acute abdomen findings. Patient was discharged with the recommendations. Due to the recurrence of abdominal pain and vomiting, the patient was admitted to our clinic because of the acute abdomen. She had hyperpigmentation in her lips and mouth and common tenderness in the abdominal examination. Intussusception was found at right upper side on abdominal USG. The patient underwent hydrostatic reduction with USG. Hydrostatic reduction didn't solve the problem so it was decided to take operation. The patient was operated. In operation, a 10 cm intussusception was seen proximal to the 150 cm of ileocaecal valve. After manual reduction, a polyp was palpated in the intestinal lumen. Other segments were controled another polyp was found. The distance between two polyps are 30 cm. Resection of 30 cm segment was decided. Operation was terminated after anastomosis. Pathologist report as hamartomatosis polyp.

Conclusion:

As seen in our patient, unexpected localized intussusception which is seen in older ages, should be investigated for etiology. It should be kept in mind that the hyperpigmentation of the oral mucosa is associated with the finding of Peutz Jeghers syndrome and the underlying GIS polymorphism.

Peutz Jeghers sendromu nedeni ile oluşan İnvajinasyon; Olgu Sunumu

Giriş ve Amaç

Peutz Jeghers sendromu, gastrointestinal sistemde hamartomatöz polipler ve mukokütanöz pigmentasyon ile karakterize otozomal dominant geçişli bir hastalıktır. İdiopatik İnvaginasyon çocuklarda 9-18 ay arasında sık karşılan bir cerrahi patoloji olmasına rağmen leading point nedeni ile oluşanlar genellikle atipik yerleşimli ve açılır kapanır tarzda görülmektedir.

Burada tekrarlayan karın ağrısı nedeni ile önceden çocuk kliniği tarafından takip edilmiş sonrasında invajinasyon nedeni ile ameliyat edilen Peutz Jeghers sendromlu bir hasta sunulmuştur.

Vaka Sunumu

6 yaşında kız hasta, son 1 aydır devam eden ve ara ara olan karın ağrısı nedeni ile pediatri kliniğinde yatırılarak tedavi edilmiş. Tekrarlayan karın ağrısı nedeni ile çocuk cerrahisi tarafından da değerlendirilen ve cerrahi patoloji tespit edilemeyen hasta önerilerle taburcu edilmiş. Karın ağrısının tekrarlaması ve kusmalarının olması nedeniyle tekrar hastaneye başvuran hasta intestinal obstrüksiyon bulguları ile kliniğimize yatırıldı. Fizik incelemede dudaklarında ve ağız içinde hiperpigmentasyon mevcuttu. Karın muayenesinde hassasiyet mevcuttu. Batın USG de sağ üst kadranda 5 cmlik invajıne segment tespit edildi. Hastaya USG eşliğinde hidrostatik redüksiyon denendi. Total redüksiyon sağlanamayan hasta ameliyat edildi. Operasyonda ileoçekal valvin 150 cm proksimalinde 10 cmlik invajinasyon görüldü. Manuel redüksiyon sonrası lümen içerisinde polip palpe edilmesi üzerine ince barsaklar palpasyonla kontrol edildi. Palpasyonda yaklaşık 30 cm distalde bir adet polip daha tespit edildi. Her iki polipi içine alacak şekilde barsak rezeksiyonu yapıldı. Histopatolojik inceleme hamartomatöz polip olarak rapor edildi.

Sonuç:

Ağız mukozası yerleşimli hiperpigmentasyonun Peutz Jeghers sendromunun bulgusu olabileceği ve beraberinde gözlenen GİS poliplerinin invajinasyon nedeni olabileceği bilinmektedir. İlk etapta tespit edilemeyen patolojilere açılır kapanır tarzda gözlenen invajinasyonlar neden olabileceği için bu hastalar yakın takipte tutulmalıdır.
