A MORTAL DISEASE WITH DERMATITIS, PERIANAL FISTULA AND DIARRHEA: INFANTILE CROHN'S DISEASE

Aim: Crohn's disease is rare in childhood and much rarer in infantile period. Dermatitis with perianal fistula and abscess may be the only finding. In this study, we aimed to present a 5-month-old patient with perianal fistula and intermittent diarrhea attacks and discuss treatment options.

Case: While a 5-month-old female patient had no complaints until 2 months ago, diarrhea attacks and weight loss began from that date. The patient was hospitalized in the external center and treated but did not improve, dermatitis developed in the perianal and genital area and perianal fistula was present for the last 1 month. The patient was referred to our center and hospitalized to our clinic. Physical examination revealed diffuse skin eruptions and perianal fistula. Enteral feeding was stopped and antibiotherapy was started. Fistulography revealed that the fistula tract was opened distally to the descending colon. Colostomy was decided and operated. Pancolitis was present and perforations were present in the transverse colon. In addition, diffuse ulceration was detected in the colon mucosa. The ileostomy was opened and the colon was repaired primary. There was also mucositis in the terminal ileum. The pathology was consistent with Crohn's disease and genetic examination was initiated. However, despite repeated operations, anastomotic leakage, multiple perforations and sepsis led to the death of the patient after 4 weeks; we couldn't started transition to enteral nutrition and treatment of Crohn's disease.

Conclusion: Infantile Crohn's disease is rare and should be kept in mind in episodes of perianal fistula, dermatitis and chronic diarrhea. Delayed diagnosis and treatment lead to grave morbidity and mortality.

DERMATİT, PERİANAL FİSTÜL VE İSHALLE GELEN MORTAL BİR HASTALIK: İNFANTİL CROHN HASTALIĞI

Amaç: Crohn hastalığı, çocukluk döneminde nadir olup,infantil dönemde çok daha nadirdir. Bez bölgesinde dermatit ile beraber perianal fistül ve apse tek bulgu olabilir. Çalışmamızda perianal fistül ve aralıklı ishal atakları olan 5 aylık bir hastayı sunmayı ve tedavi seçeneklerini tartışmayı amaçladık.

Olgu: 5 aylık bir kız hastada, 2 ay öncesine kadar hiçbir şikayeti yok iken o tarihten itibaren ishal atakları ve kilo kaybı başlıyor. Dış merkezde yatırılıp tedavi verilen ancak kliniğinde düzelme olmayan, perianal ve genital bölgede dermatit gelişen hastada, son 1 aydır perianal fistül gelişiyor. Hasta bu tabloyla merkezimize sevk edildi ve kliniğimize yatırıldı. Muayenesinde yaygın cilt döküntüleri ve perianal fistül mevcuttu. Hastanın enteral beslenmesi kesildi ve antibiyoterapi başlandı. Fistülografide fistül traktının inen kolon distaline açıldığı gözlendi. Kolostomiye karar verildi ve ameliyata alındı. Tüm kolon segmentinde pankolit hali mevcuttu ve transvers kolonda perforasyonlar mevcuttu. Ayrıca kolon mukozasında yaygın ülserasyonlar saptandı. İleostomi açıldı ve kolon primer tamir edildi. Terminal ileumda da mukozit tablosu vardı. Patoloji Crohn ile uyumlu olup genetik inceleme başlatıldı. Ancak mükerrer ameliyatlara rağmen anastomoz kaçağı, multipl perforasyonlar ve sepsis nedeniyle enteral beslenmeye geçilemeden ve Crohn’a yönelik tedavi başlanamadan 4 hafta sonunda hasta kaybedildi.

Sonuç: İnfantil Crohn hastalığı nadir olup perianal fistül, dermatit ve kronik ishal ataklarında akılda tutulması gereken bir tablodur. Geciken tanı ve tedavi ciddi morbidite ve mortaliteye yol açmaktadır.
