Pediatric femoral hernia associated with fibrolipoma: a case report and review of literature

Femoral hernia is a protrusion of the peritoneal sac through the femoral canal. It accounts for less than 1% of all groin herniae in children. Preoperative misdiagnosis of 40 to 75% in patients with femoral herniae stems from the confusion with other inguinal pathologies including inguinal hernia, lymphadenitis, hydrocoele or others. Insufficient physical examination and inadequate experience about childhood femoral herniae may also play a role in misdiagnosis and therapeutic failure. With a correct diagnosis and an appropriate surgical management, it is possible to prevent possible complications such as intestinal necrosis and perforation due to strangulation of the involved viscera even mortality from delayed presentation and treatment. A 5-year-old boy with a femoral hernia associated with a lipomateous mass at the tip of hernia sac was surgically treated. The histopathological examination of the excised specimen revealed a fibrolipoma. To the best of our knowledge, this is the first pediatric report of a combination of femoral hernia and fibrolipoma. Femoral hernia and fibrolipoma may coexist in the same patient and total excision is the gold standard for diagnosing fibrolipoma. This information adds to the literature that femoral hernia and fibrolipoma can be seen together as distinct entities in children and should be treated accordingly.

Fibrolipomun eşlik ettiği femoral herni: olgu sunumu ve literatür değerlendirmesi

Femoral herni peritoneal kesenin femoral kanaldan protrüzyonu sonucu meydana gelir. Çocukluk çağı kasık fıtıklarının %1'den azını oluşturur. İnguinal herni, lenfadenit, hidrosel ve diğer kasık patolojileri ile karışabildiğinden ameliyat öncesi yanlış tanı oranı %40 ile %75 arasında değişmektedir. Yetersiz fizik muayene ve çocukluk çağı femoral herni hakkında eksik deneyim gerek yanlış tanı gerekse tedavi başarısızlığında rol oynayabilir. Doğru tanı ve uygun tedavi ile strangulasyonun neden olabileceği intestinal nekroz, barsak perforasyonu gibi komplikasyonlardan kaçınılabilir, geç başvuru ve geç tedavi sonrası meydana gelebilecek mortalitenin de önüne geçilebilir. Beş yaşında erkek olgu herni kesesinin ucunda lipomatöz kitlenin eşlik ettiği femoral herni nedeniyle opere edildi. Eksize edilen kitlenin histopatolojik incelemesi fibrolipoma olarak rapor edildi. Literatür araştırması sonucu sunulan olgunun femoral herni ve fibrolipom birlikteliği olan ilk pediatrik hasta olduğu kanısında varıldı. Aynı hastada femoral herni ve fibrolipom birlikte görülebilir. Fibrolipoma tanısında total eksizyon altın standart olarak kabul edilmektedir. Sunulan olgu ile çocuklarda femoral herni ve fibrolipomun birlikte görülebileceği ve bu olguların uygun şekilde tedavi edilmeleri gerektiği kanısına varılmıştır.
