EVALUATION OF TWO DIFFERENT MINIMAL INVASIVE TECHNIQUES IN THE TREATMENT OF BLADDER STONE IN CHILDREN

İntroduction: Bladder stones in the pediatric age group are more common in developing countries. Parallel to recent technological advances, there have been some changes in the minimally invasive approach to bladder stone treatment. Our aim in this study; In our clinic, we compare the results of two different minimally invasive treatment modalities.

Material and Method: The records of 43 pediatric patients (40 boys, 3 girls) who underwent endoscopic surgery for bladder stone between January 2012 and December 2018 were retrospectively analyzed. Patients were divided into two groups according to the intervention type. Twenty-three patients who underwent Holurium-YAG laser transurethral Cystolithotripsy and 20 patients who underwent Mini Percutaneous Cystolithotomy. Demographic data, operation time, hospital stay, post-operative complications were recorded and 2 groups were compared.

Results: The mean age was 49.1 months (7 - 216) and there was no significant difference between the two groups. The mean stone size was 16.2 mm (7 - 30) and there was no significant difference between the two groups. The mean operation time was 37 minutes (20 - 60) and there was no significant difference between the two groups. The mean hospital stay was 2 days (1-6) in the first group and 3.7 days (2-10) in the second group, and the difference was statistically significant (p <0.05). All patients were free of stones. In the second group, urinary extravasation occurred at the entrance of the trocar in 2 patients. In both groups, 2 patients were treated with postoperative fever and urinary tract infection. Minor complications such as dysuria and hematuria were similar in both groups.

Conclusion: Both methods are successful in the treatment of bladder stones less than 3 cm. Transurethral Cystolithotripsy with Holmium-YAG laser is a less invasive procedure and it is a more advantageous method in terms of shortening the length of hospital stay.

ÇOCUKLARDA MESANE TAŞI TEDAVİSİNDE İKİ FARKLI MİNİMAL İNVAZİF TEKNİĞİN DEĞERLENDİRİLMESİ

Giriş ve Amaç: Pediatrik yaş grubundaki mesane taşlarına gelişmekte olan ülkelerde daha sık rastlanmaktadır. Son yıllardaki teknolojik gelişmelere paralel olarak mesane taşı tedavisindeki minimal invaziv yaklaşımda da birtakım değişimler olmuştur. Bu çalışmadaki amacımız; kliniğimizde uygulanan 2 farklı minimal invaziv tedavi şeklinin sonuçlarının karşılaştırılmasıdır.

Gereç ve Yöntem: Ocak 2012 ile aralık 2018 tarihleri arasında mesane taşı nedeniyle endoskopik cerrahi tedavi uygulanmış 43 çocuk hastanın(40 Erkek, 3 Kız) kayıtları geriye dönük olarak incelendi. Müdahale şekline göre hastalar 2 gruba ayrıldı. Birinci grup Holmiyum-YAG lazerle Transüretral Sistolitotripsi uygulanan 23 hasta, İkinci grup ise Mini Perkütan Sistolitotomi uygulanan 20 hasta idi. Demografik bilgiler, operasyon süresi, hastanede kalış süresi, post-operatif komplikasyonlar kaydedildi ve 2 grup karşılaştırıldı.

Bulgular: Ortalama yaş 49,1 ay (7 - 216) olup her iki grup arasında anlamlı fark saptanmadı. Ortalama taş boyutu 16,2 mm (7 - 30) olup her iki grup arasında anlamlı fark saptanmadı. Ortalama operasyon süresi 37 dakika (20 - 60) olup her iki grup arasında anlamlı fark saptanmadı. Ortalama hastanede kalış süresi birinci grupta 2 gün (1-6) , ikinci grupta ise 3,7 gün (2-10) olup, aradaki fark istatistiksel olarak anlamlı bulundu (p<0,05). Tüm hastalarda taşsızlık sağlandı. İkinci grupta 2 hastada trokar giriş yerinden üriner ekstravazasyon gelişti ve foley sonda konularak kapanması beklendi. Her iki gruptada 2’şer hastaya postoperatif ateş yüksekliği ve idrar yolu enfeksiyonu nedeniyle tedavi verildi. Operasyon sonrası dizüri ve hematüri gibi minör komplikasyonlar her iki grup arasında benzerdi.

Sonuç: Her iki yöntemde 3 cm’nin altındaki mesane taşlarının tedavisinde başarılıdır. Holmiyum-YAG lazer ile yapılan transüretral sistolitotripsi, daha az invaziv bir işlem olması ve hastanede kalış süresini kısaltması açısından daha avantajlı bir yöntem olarak öne çıkmaktadır.
