Anoplasty for the Treatment of Anorectal Malformation in Newborn: Attention! Rectobulbar Fistula

The incidence of anorectal malformations is 1/5000 live births and only 5% of the cases have no fistula. ARM with rectourethral fistula is the most common type in boys. Here we present a boy with rectourethral fistula who had underwent anoplasty during the newborn period as he was regarded as low type ARM.

A 38 days old boy with a history of anoplasty performed at the age of 1 day old in another center was admitted to our department for faecal discharge from the urethra for 20 days. On physical examination, location of the anus was normal but was significantly narrow. Retrograde urethrography is performed and a fistula between the bulbar urethra and anterior wall of the distal rectum is identified. Leukocytosis and urinary tract infection is found on laboratory tests. The boy had underwent diverting sigmoidostomy and anal dilation program was initiated. After achieving sufficient anal calibration, examination under general anesthesia is performed and anus location was found slightly anterior. On cystourethroscopy, a rectourethral fistula with a wide opening is found and proximal urethra had a narrow angled course. After insertion of a uretheral catheter through the fistula, other opening of the fistula is demonstrated on the anterior part of the rectum, 1.5 cm proximal to the anus by means of an endoscopy inserted through the sigmoidostomy. Anterior sagittal incision is performed for surgical exploration and found that urethra was pulled to the subcutaneous region of the perineum due to traction by pull-through of the rectum. After separation of the fistula from rectum and urethra, urethral and rectal wall is repaired. On control cystourethroscopy 1 months later, urethra was normal and sigmoidostomy was closed. No complication or morbidity is encountered during 5 year follow up.

Newborns with ARM should be closely monitored during the first 24 hours of life to identify urethral meconium discharge after swallowed air can reach rectum. If no sign of fistula is identified after 24 hours, lateral cross table graphy should be obtained and rectal air and possibility of fistula should be evaluated cautiously.

Yenidoğanda Anorektal Malformasyon Tedavisinde Anoplasti: Dikkat! Rektobulber Fistül

Anorektal malformasyonlar (ARM) 5000 doğumda bir görülür ve olguların sadece %5'inde fistül yoktur. Erkek çocuklarda en sık rektoüretral fistüllü ARM görülür. Burada perineye yakın olduğu düşünülerek yenidoğan döneminde anoplasti yapılan, rektoüretral fistüllü bir olgu sunulmuştur.

Anal atrezi nedeniyle 1 günlükken dış merkezde anoplasti yapılmış olan 38 günlük erkek bebek, 20 gündür idrarından aralıklı kaka gelmesi şikayetiyle başvurdu. Hastanın muayenesinde anüs normal lokalizasyonda ve oldukça dardı. Retrograd üretrografide; bulber üretradan başlayıp, rektum distali anterior duvarına uzanan fistül saptandı. Laboratuvar tetkiklerinde lökositoz ve idrar yolu enfeksiyonu mevcuttu. Hastaya diverjan sigmoid kolostomi açılıp, anal dilatasyon programı başlandı. Yeterli anal dilatasyon sağlandıktan sonra genel anestezi altında sfinkter muayenesi yapıldı, anüsün hafif anterior yerleşimli olduğu görüldü. Sistoüretroskopide bulber üretrada geniş bir rektoüretral fistül olduğu, fistülün proksimalindeki üretranın dik bir açıyla yukarıya doğru ilerlediği görüldü, üreteral stent fistülden rektuma ilerletildi. Daha sonra bükülebilir endoskop ile rektoskopi yapıldı, anüse yaklaşık 1,5 cm mesafede önde fistül girişi saptandı. Anterior sagital insizyon ile yapılan eksplorasyonda; üretranın fistül nedeniyle rektum ile birlikte cilt altına doğru çekilmiş olduğu görüldü. Transanal yoldan fistülün rektum tarafı, rektum duvarından ayrılarak serbestleştirilip perineal insizyon bölgesine alındı. Fistül bulber üretradan da ayrıldıktan sonra rektum ve üretra duvarı onarıldı. 1 ay sonra kontrol sistoüretroskopisi normal olan hastanın sigmoid kolostomisi kapatıldı. Hastanın 5 yıllık takiplerinde sorun olmadı.

ARM'li yenidoğanlar yutulan havanın rektuma ulaşması ve fistül yoluyla mekonyum drenajının görülebilmesi açısından doğumdan sonraki ilk 24 saat boyunca yakın takip edilmelidir. 24 saat sonunda fistül bulgusu yoksa çapraz masa lateral grafi çekilerek, rektumdaki havanın yerleşimi ve fistül olasılığı dikkatli bir şekilde değerlendirilmelidir.
