Clinical experience of 21 years in hypospadias surgery

Purpose: We aimed to evaluate the surgical treatments performed in patients with hypospadias and the associated complications.

Materials and Methods: Totally, 316 patients who underwent surgery for hypospadias between 1998 and 2019 were examined with respect to their age at surgery, localisation of urethral meatus, surgical method used, and complications. The patients were divided into three groups according to the localisation of the urethral meatus as distal (glanular, coronal, subcoronal), penile (distal penile, midshaft, proximal penile) and proximal (penoscrotal, scrotal, perineal) and into four groups according to the age at surgery (≤24 months, 25–60 months, 61–120 months, 121–192 months).

Results: The median age was 51 months (1–192 months). Further, 64.4% of the patients had distal, 17.5% had penile and 18.1% had proximal hypospadias. The Snodgrass, MAGPI, Mathieu and Duckett methods were used in the decreasing order of frequency. Complications occurred in 20.3% of the patients (stenosis: 15.2%, fistula: 9.2%, total dehiscence: 1.9%). A multi-stage surgical procedure was performed in 15.5% of the 58 patients with proximal hypospadias. Stenosis was the only complication observed in 3 patients who underwent multi-stage surgery, and dilatation was sufficient to treat. The rate of complication was 51% in patients who did not undergo multi-stage surgery. With respect to the occurrence of complications, there was a statistically significant difference between the patients with proximal hypospadias and the patients with distal and penile hypospadias (p = 0.000). According to their age at surgery, 28.2% of the patients were in group 1, 29.1% were in group 2, 34.8% were in group 3 and 7.9% were in group 4. The rates of complications were 18%, 22.8%, 19.1% and 24%, respectively (p = 0.808). Stenosis occurred in 3.7% of patients who underwent dilatation in the early postoperative period (51.6% of patients) and in 30.1% of the patients who did not undergo dilation (48.4% of patients) (p = 0.000).

Conclusion: In patients with proximal hypospadias, the rate of complications can be reduced by performing multi-stage surgery. The age at surgery was not found to have an effect on the rate of complications. Dilatation performed in the early postoperative period can help reduce the rate of complications.

Hipospadias cerrahisinde 21 yıllık klinik deneyim

Amaç: Hipospadias olgularına uygulanan cerrahi tedavi ve komplikasyonlarının irdelenmesi amaçlanmıştır.

Gereç ve Yöntem: 1998–2019 yılları arasında hipospadias nedeniyle ameliyat edilen 316 hasta ameliyat yaşı, üretral mea lokalizasyonu, uygulanan cerrahi yöntem, komplikasyonlar ve takip süreleri açısından incelendi. Olgular üretral meanın lokalizasyonuna göre distal (glanüler, koronal ve subkoronal), penil (distal penil, midşaft ve proksimal penil) ve proksimal (penoskrotal, skrotal ve perineal) olarak 3, ameliyat yaşlarına göre (≤24 ay, 25-60 ay, 61-120 ay, 121-192 ay) 4 gruba ayrıldı.

Bulgular: Ortanca yaş değeri 51 (min:1, max:192) aydı. Hastaların %64,4’ü distal, %17,5’i penil, %18,1’i proksimal hipospadiasdı. Olgulara sıklık sırasına göre Snodgrass, MAGPI, Mathieu ve Duckett yöntemleri kullanıldı. Olguların %20,3’ünde komplikasyon (darlık: %15,2, fistül: %9,2, total açılma: %1,9) gelişti. Proksimal yerleşimli 58 olgunun %15,5’ine aşamalı cerrahi işlem uygulandı. Aşamalı cerrahi yapılan olgularda sadece darlık (n:3) gelişti ve tedavide dilatasyon yeterli oldu. Aşamalı cerrahi yapılmayan olguların komplikasyon oranı %51 idi. Komplikasyon gelişimi açısından, proksimal yerleşimli olgular ile distal ve penil yerleşimli olgular arasında istatistiksel olarak anlamlı fark vardı (p=0.000). Yaşa göre olguların %28,2’si grup 1, %29,1’i grup 2, %34,8’i grup 3 ve %7,9’u grup 4 idi. Komplikasyon oranları sırasıyla %18, %22,8, %19,1 ve %24 idi (p = 0.808). Ameliyat sonrası erken dönemde dilatasyon yapılan (%51,6) olguların %3,7’sinde, yapılmayanların (%48,4) ise %30,1’inde darlık gelişti (p=0.000). Darlık gelişen olgular dilatasyonla düzeldi.

Sonuç: Proksimal hipospadias olgularında aşamalı cerrahi ile komplikasyon oranları azaltılabilir. Ameliyat yaşının komplikasyonlar üzerine etkisi görülmedi. Ameliyat sonrası erken dönemde yapılan dilatasyon/kalibrasyon komplikasyon oranını düşürebilir.
