OUR OVARIAN TORSION EXPERIENCE

Objective:Ovarian torsion is a rare gynecological emergency that may cause serious consequences.Doppler ultrasound is the first and the most reliable method in the diagnosis. Rapid diagnosis and surgical treatment are the basis for protecting of the ovary.In this study,we aimed to present the ovarian torsion cases diagnosed with physical examination and ultrasound findings and operated in our clinic.

Materials and Methods:Patients who underwent surgical treatment for ovarian torsion in our clinic between2013 and 2017 were retrospectively analyzed.Patients were evaluated in terms of age,history,examination findings,diagnostic methods,surgical findings and postoperative follow-up.

Results:The mean age of the 17patients operated in our clinic was 10.8(0-18).All patients underwent preoperative doppler ultrasound and were diagnosed with ovarian torsion.4 patients who were diagnosed with ovarian torsion on ultrasound were not included in the study because torsion was not seen intraoperatively.The diagnosis of 4patients was confirmed by MRI because of suspicious findings on ultrasound.13patients had pathologic ovarian cysts with dimensions between 3-10cm and all of them were simple cysts.Laparoscopic(6)or open surgery(11)was applied to all patients.Oophorectomy was performed in 4patients,ovarian detorsion and unilateral fixation was performed in the remaining 11patients.In 2patients,ovarian detorsion without fixation was performed.No fixation performed to the other healthy ovary in none of the patients.Cyst excision or aspiration and decapitonage were performed in the ovaries with cysts; or only follow-up.The ovaries were rotated between 1.5-4turns.In all of the patients who underwent surgery in the first 24hours or who had rotation 2turns and below, control ultrasounds 3months later were normal.1-4days after the operation,all patients were discharged without any problem.

Conclusion:Although the incidence of ovarian torsion increases in child age group,especially in the adolescent period,the diagnosis may be delayed due to nonspecific physical examination findings.The delay of surgery and the degree of torsion appear to be the two most important parameters determining the prognosis.

Over Torsiyonu Deneyimlerimiz

Amaç: Over torsiyonu(OT) nadir görülen ancak ciddi sonuçlara yol açabilecek jinekolojik acillerden biridir. OT tanısı her zaman kolay koyulamamaktadır.Tanıda dopler USG ilk başvurulan ve en güvenilir yöntemdir.Hızlı tanı ve cerrahi tedavi, hastanın overinin korunmasının temelidir. Klinik ve USG ile OT tanısı almış ve opere edilmiş olgularımızın sunulması amaçlandı .

Materyal ve Metod: Kliniğimizde 2013 ile 2017 yılları arasında OT nedeniyle cerrahi tedavi uygulanan hastalar geriye dönük olarak dosyaları incelendi. Opere edilen olgular yaş, öykü, muayene bulguları, tanı yöntemleri, ameliyat bulguları ve ameliyat sonrası takipleri açısından değerlendirildi.

Bulgular: Kliniğimizde son 5 yılda opere edilen 17 hastanın yaşları ort 10,8 ( 0-18) idi. Hastaların hepsine preoperatif olarak Dopler USG yapılmış ve over torsiyonu tanısı almış idi. USG de OT tanısı almış olan 4 hasta intraoperatif olarak torsiyon olmadığı için çalışmaya dahil edilmedi. 4hastanın USG de şüpheli bulgular nedeniyle MR çekilmiş ve tanı doğrulanmıştır. 13 hastada boyutları 3/ 10cm arasında olan patolojik kist mevcuttu ve patolojik kist tamamı basit kistti. Hastaların tamamına acil olarak laparaskopik(6) ya da açık cerrahi(11) uygulanmıştır. Hastaların 4 tanesine ooferektomi uygulanmış kalan 11 hastaya over detorsiyonu +fiksasyon , 2 hastaya ise sadece over detorsiyonu uygulanmış over fiksasyonu yapılmamıştır. Sağlam olan diğer over fiksasyonu hiçbir hastada yapılmamıştır. Kisti bulunan overlerde kist eksizyonu , aspirasyonu + dekapitonaj yada takip uygulanmıştır. Overlerin 1,5 /4 tur arasında döndüğü gözlenmiştir. İlk 24 saattte cerrahi uygulanan veya torsiyon oranının 2 tur ve altında tespit edilen hastaların tamamında 3 ay sonra çekilen kontrol usg de over kanlanmasının ve boyutlarının normal olduğu gözlenmiştir. Operasyondan sonra hastaların tamamı 1- 4 gün arasında sorunsuz olarak taburcu edilmiştir.

Sonuç: Over torsiyonunun çocukluk yaş grubunda özellikle adelosan dönemde artmakla beraber her hangi bir yaş grubunda non spesifik FM bulguları nedeniyle tanısı gecikebilmekte ve cerrahi zamanlamanın gecikmesi ise torsiyonun derecesi ile beraber prognozu belirleyen en önemli 2 parametre gibi görünmektedir.
