Neonatal Gastric Perforation: Report of 11 Cases

Introduction:

Neonatal gastric perforation is a rare entity and lifethreatening condition. In this retrospective study, we reviewed all patients with neonatal gastric perforations who underwent surgery at a pediatric tertiary hospital during a 9-year period. The aim of this study was to review the clinical course of neonatal gastric perforation and to evaluate possible prognostic factors.

Patients and methods:

From January 2010 to February 2019, eleven patients were diagnosed as gastric perforations in our institution. Mean birth week: 29.8 (26-38weeks), birth weight: 1196gram (710-3080gram), 6 boys, 5 girls newborn. 9 cesarean section, 2 vaginal births. The average day of perforation: 6th day (1-10 days). Three patients (27.2%) died (postoperative 8, 34, 84 days). One patient had diaphragmatic hernia (Table)

Perforation was observed in 4 (36.3%) gastric anterior, 3 (27.2%) large curvature, 2 (18.1%) posterior, small curvature in 1, total perforation in 1 patient. Two patients had intestinal adhesion and two patients were reoperated for intestinal perforation. Perforation size ranged from 0.2mm to 8cm, with an average of 4.6 cm. Prematurity and preeklampsi have recently been shown to be a common feature in spontaneous gastric perforation.

Discussion:

The etiology of neonatal gastric perforation is still unclear. Several possibilities have been discussed, including congenital defects of the gastric muscle wall mechanical disruption stress ulceration secondary to neurogenic disorders and ischemia of the gastric wall secondary to vascular shunting.

Among the risk factors we investigated, gestational age, birth weight, associated anomaly, seem to have significant prognostic impact on the overall mortality. Most of the neonatal gastric perforations occur within the first week of life.

Conclusion:

In summary, the mortality associated with neonatal gastric perforation is high, particularly in premature, LBW neonates. Mortality is inversely related to birth weight. It is hoped that early diagnosis and early intervention may improve the prognosis.

Giriş:

Yenidoğan gastrik perforasyonu nadir görülen bir hastalıktır ve yaşamı tehdit eden bir durumdur. Bu çalışmada, 9 yıllık bir süre içerisinde pediatrik bir üçüncü basamak hastanede ameliyat edilen neonatal gastrik perforasyonu olan tüm hastaları inceledik. Bu çalışmanın amacı yenidoğan mide perforasyonunun ve olası prognostik faktörlerin değerlendirilmesidir.

Materyal ve method:

Kliniğimizde Ocak 2010- Şubat 2019 tarihleri arasında ameliyat edilen mide perforasyonlu 11 olgunun verileri incelendi. Ortalama doğum haftası:29,8(26-38hafta), doğum kilosu:1196gram(710-3080gram), 6 erkek, 5 kız yenidoğan. 9 sezeryan, 2 vajinal doğum. Perforasyonun görüldüğü ortalama gün:6.gün(1-10.gün). 3 olgu(%27,2) kaybedildi(ameliyat sonrası 8, 34, 84 günler). Bir olgu diyafram hernisi ile birlikteydi. (Tablo)

Perforasyon 4 olguda(%36,3) mide önyüzünde, 3 olguda(%27,2) büyük kurvatur, 2 olguda(%18,1) arka yüzde, 1 olguda küçük kurvatur, 1 olguda total perforasyon gözlendi. 2 hasta barsak yapışıklığı 2 hasta da intestinal perforasyon nedeniyle tekrar ameliyat edildi. Perforasyon büyüklüğü 0,2mm-8cm arasında, ortalama 4,6 cm olarak bulundu. Prematürite ve preeklampsi en sık mide perforasyonu nedeni olarak görülmektedir.

Tartışma:

Yenidoğan gastrik perforasyonunun etiyolojisi hala belirsizdir. Nörojenik hastalıklara sekonder gastrik kas duvarı mekanik bozulma, stres ülseri ve vasküler şanta sekonder gastrik duvarın iskemisi gibi çeşitli olasılıklar söz konusudur.

Risk faktörleri araştırıldığında gebelik sorunları, gebelik haftası, doğum ağırlığı, ek anomali gibi faktörlerin prognozda ve sağ kalımda önemli olduğu görüldü. Perforasyonların büyük çoğunluğu hayatın ilk haftası içinde görüldü.

Sonuç:

Mide perforasyonu erken doğan ve düşük doğum ağırlıklı yenidoğanlarda ciddi mortalite nedenidir. Erken tanı ve erken girişim önemlidir.
