Modified Nuss Procedure

Modified Nuss Procedure A minimally invasive procedure, described by Donald Nuss in 1987 for the repair of pectus

excavatum, is a minimally invasive method performed with a guided substernal bar with two thoracic incisions without

cartilage and sternum resection; has undergone several modifications by surgeons over the years and continues to undergo.

Although rare, they cause life-threatening conditions and these modifications are about ; incision sites, bar size and

placement, fixation and passage of the bar. Tuneloscopy during the passage of the bar is argued to prevent cardiac and

vascular injury. In order to facilitate the surgeon's passage through the substernal area and to prevent complications, we

made a tunnel with an endoscopic 5mm-curved dissector from the retrosternal tissue between the pericardium and sternum

with a thoracoscopy, and performed a tunnel-to-hemithorax-guided ip-guide placement. With this modification, it was

ensured that the bar was placed in an appropriate and safe way with the guidance. We will present the details of the

modification that we emphasize the ease of dissection during tuneloscopy.

Modifiye Nuss Prosedürü

Pektus ekscavatum onarımında 1987’ de Donald Nuss tarafından tanımlanan ve kartilaj ve sternum rezeksiyonu yapılmaksızın iki taraftan yapılan torasik insizyonlarla guide eşliğinde substernal bar yerleştirilerek yapılan minimal invaziv yöntem olan Nuss Prosedürü; yıllar içinde cerrahlar tarafından çeşitli modifikasyonlar geçirmiş ve geçirmeye de devam etmektedir. Nadir görülse de hayatı tehdit edebilecek durumlara sebebiyet vermesi ile komplikasyonlara yönelik bu modifikasyonlar; insizyon yerleri, bar boyutu ve yerleştirilme şekli, sabitlenmesi ve barın geçişi ile ilgilidir. Barın geçişi sırasında tuneloskopinin kardiak ve damar yaralanmasını önlediği savunulmaktadır. Biz cerrahın güvenle substernal alandan geçişini kolaylaştırmak, komplikasyonları önlemek için torakoskopi eşliğinde perikard ile sternum arasındaki retrosternal dokudan endoskopik 5mm-curved disektör ile tünel oluşturulup tuneloskopi eşliğinde karşı hemitoraksa geçilerek ip-guide yerleştirme işlemini gerçekleştirdik. Bu modifikasyon ile guide eşliğinde barı uygun ve güvenli şekilde yerleştirilmesi sağlandı. Tuneloskopi sırasındaki diseksiyonun kolaylığını vurguladığımız modifikasyonun ayrıntılarını video şeklinde sunacağız.
