Kidney transplant results in child age group: single center experience

Objective: We evaluated the results of patients under the age of 18 who underwent renal transplantation in our center for end-stage renal disease (ESRD). Methods: Patients under 18 years of age who underwent renal transplantation between 2010 and 2018 were evaluated retrospectively in terms of demographic data, post-transplant complications, graft and patient survival.

Results: Of the 43 patients who underwent renal transplantation, 20 (46.5%) were female and 23 (53.5%) were male. The mean age of transplantation was 13.5 ± 3.7 (2-18) years. All patients underwent live donor kidney transplantation. Right kidney transplantation was performed in 12 (27.9%) cases. When the causes of ESRD were examined, 13 patients had primary insufficiency, 8 patients had vesicourethral reflux nephropathy and 7 patients had nephritic findings. In the pre-transplant period, 20 patients were receiving hemodialysis and 8 patients were receiving renal replacement therapy with peritoneal dialysis. Fifteen patients were premature. Mean hospital stay was 7.5 ± 2.7 days and mean follow-up was 42.8 ± 30.1 months. When the early graft function was evaluated, the mean creatinine value was 0.83 ± 0.48 mg / dl on the fifth day and 0.92 ± 0.25 mg / dl at the end of the first year. None of the patients had delayed graft function. During the follow-up period, one patient died due to viral infection five months after transplantation, five patients returned to dialysis and four patients were lost to follow-up.

Conclusion: Renal replacement therapy in ESRD patients should be the primary target of renal transplantation in all age groups. When applied in experienced centers, renal transplantation, long graft and patient survival is the best treatment model in pediatric age groupBçbrek

Çocuk yaş grubunda böbrek nakli sonuçlarımız: tek merkez deneyimi

Amaç: Son dönem böbrek yetmezliği (SDBY) nedeni ile merkezimizde böbrek nakli uygulanan 18 yaş altı hastaların sonuçlarını değerlendirdik.
Yöntem: 2010- 2018 yılları arasında böbrek nakli yapılan 18 yaş altı hastalar demografik veriler, nakil sonrası komplikasyonlar, greft ve hasta sağ kalımı açısından retrospektif olarak değerlendirildi.

Bulgular: Böbrek nakli uygulanan toplam 43 hastanın 20'si (%46,5) kadın, 23'ü (%53,5) erkekti. Ortalama nakil yaşı 13,5±3,7 ( 2-18) yıldı. Tüm olgular canlı vericili böbrek nakli oldu. 12 (%27,9) olguya sağ böbrek nakledildi. Olguların SDBY nedenleri incelendiğinde, 13 hastada primeri bilinmeyen yetmezlik, 8 hastada vezikoüretral reflü nefropatisi, 7 hastada nefritik bulgular mevcuttu. Nakil öncesi dönemde, 20 hasta hemodiyaliz, 8 hasta periton diyalizi ile renal replasman tedavisi almaktaydı. 15 hasta ise premptifti. Olguların ortalama hastane kalış süresi 7,5±2,7 gün, ortalama takip süresi 42,8±30,1 aydı. Erken dönem greft fonksiyonu değerlendirildiğinde, ortalama kreatinin değeri beşinci gün 0,83±0,48 mg/dl, birinci yıl sonu 0,92±0,25 mg/dl idi. Hiçbir hastada gecikmiş greft fonksiyonu görülmedi. Takip süresi içinde bir hasta viral enfeksiyona bağlı olarak nakilden beş ay sonra öldü, beş hasta diyalize geri döndü, dört hasta takipten çıktı.

Sonuç: SDBY hastalarında uygulanan renal replasman tedavisinin tüm yaş gruplarında öncelikli hedefi böbrek nakli olmalıdır. Deneyimli merkezlerde uygulandığında pediatrik yaş grubunda böbrek nakli, uzun greft ve hasta sağ kalım süresi ile en iyi sonuç veren tedavi modelidir.
