Effectiveness of single dose steroid therapy in intussusception

Introduction: Recently treatment of intussusception can be provided with single dose steroid treatment that has proven to be effective and non-invasive without hospitalization.

Methods: 115 patients who have had hydrostatic reduction and 37 patients who have had steroid therapy as intussusception treatment (92 boys, 47 girls) in our hospital between 2009 and 2019 were examined.

Conclusion: The median age was 26 months (3-127 months) and the complaint duration was 12 hours (1-144 hours). The hydrostatic reduction was mostly selected in patients who have had sensitivity and/or mass in physical examination (p=0.023; p=0.014). The presence of air-fluid levels in the direct abdominal x-ray and the reporting of intussusception in ultrasound as ileocolic or ileoileal did not change the treatment selection (p=0.355; p=0.544). The presence of air-fluid levels in the direct x-ray increase treatment failure from 19.1% to 45.8% (p=0.010). Failure was found to be 10.8% with steroid treatment and 29.4% with hydrostatic reduction (p=0.026). In 87 patients who underwent hydrostatic reduction and whose reports were reached, the interruption was in 14.9% cecum, 4.6% ascending colon, 21.8% hepatic flexure, 10.3% transverse colon, 2.3% splenic flexure, 2.3% descending colon, 4.6% sigmoid colon. In 34 of the patients who have had hydrostatic reduction (39.1%), the colon and ileum were filled without interruption, and the intussusception was thought to be ileoileal. The hydrostatic reduction at 79.4% of these patients was successful and no intussusception was detected in the ultrasound control. 38 of the 39 patients who underwent steroid treatment were given steroid dosage of 1mg/kg. In 71.8% of these patients, a single dose was sufficient, while 28.2% had 2 doses of steroids either from the beginning or due to recurrence. 27% of patients who have had steroid given were hospitalized, while 100% of patients who have underwent hydrostatic reduction were hospitalized. The recurrence was 8.3% with steroid treatment and 21.3% with hydrostatic reduction (p=0.110).

Conclusion: The hydrostatic reduction was preferred in patients who have had sensitivity and/or mass in physical examination. Intravenous single-dose steroid administration is non-invasive, more effective than hydrostatic reduction and removes the need for hospitalization.

İnvajinasyonda tek doz steroid tedavisinin etkinliği

Giriş: İnvajinasyonnun klsik tedavisi girişmsel işlemleri içermektedir. Son dönemde etkinliği kanıtlanmış, non-invasiv tek doz steroid uygulaması ile yatışa gerek kalmadan tedavi sağlanabilmektedir.

Yöntem: Hastanemizde 2009-2019 yılları arasında invajinasyon tanısı alan hastalardan hidrostatik redüksiyon yapılan 115 hasta ile steroid tedavisi uygulanan 37 hastanın (92 erkek, 47 kız) dosyaları taranarak verileri incelenmiştir.

Sonuç: Hastalarımızın başvuru yaşı ortanca 26 ay (3-127 ay), şikayet süresi ortanca 12 saattir (1-144 saat). Fizik muayenede hassasiyet olan ve ele gelen kitlesi olan hastaların tedavilerinde çoğunlukla hidrostatik redüksiyon seçilmiştir (p=0,023; p=0,014). ADKG’de seviye olması ve invajinasyonun ileokolik veya ileoileal olarak raporlanması tedaviyi değiştirmemiştir (p=0,355; p=0,544). ADKG’de hava-sıvı seviyesi olması tedavi başarısızlığını %19,1’den %45,8’e çıkartmaktadır (p=0,010). Başarısızlık, steroid tedavisiyle %10,8 iken, hidrostatik redüksiyonla %29,4 olarak saptanmıştır (p=0.026). Hidrostatik redüksiyon yapılan ve raporlarına ulaşılan 87 hastada takılma %14,9 çekumda, %4,6 çıkan kolonda, %21,8 hepatik fleksurada, %10,3 transvers kolonda, %2,3 splenik fleksurada, %2,3 inen kolonda, %4,6 sigmoid kolonda olmuştur. Hidrostatik redüksiyon yapılan hastaların 34’ünde (%39,1) takılma olmadan kolon ve ileum dolmuş, invajinasyonun ileoileal olduğu düşünülmüştür. Bu hastaların %79,4’ünde başarı sağlanmış, kontrol ultrasonda invajinasyon saptanmamıştır. Steroid uygulanan 39 hastanın 38’ine 1mg/kg’dan steroid verilmiştir. Bu hastaların %71,8’inde tek doz yeterli olurken, %28,2’sine baştan veya tekrarlama nedeniyle 2 doz steroid yapılmıştır. Steroid verilen hastaların sadece %27’sinde yatışa olurken, hidrostatik redüksiyon yapılan hastaların %100’ünde yatış gerekli olmuştur. Nihayi tedavi olarak steroid verildiğinde %8,3 tekrarlama görülürken, hidrostatik redüksiyon sonrası %21,3 tekrarlama görülmüştür (p=0,110).

Sonuç: Fizik muayenede hassasiyet ve/veya ele gelen kitlesi olan hastalarda hidrostatik redüksiyon tercih edilmiştir. Bu bulguları daha ciddi bir klinik ile eşleştirilerek ileri bir tedavi tercih edildiğini düşünmekteyiz. İnvajinasyon tanısı almış hastalarda IV tek doz steroid uygulaması, non-invasiv, yatış gerektirmeyen, hidrostatik redüksiyondan daha etkin bir tedavi şeklidir.
