A rare complication ofpercutaneous endoscopic gastrostomy (PEG): trans-ileal perforation and posterior wall gastrostomy

Introduction: Percutaneous endoscopic gastrostomy (PEG) in patients with long term nutritional problems due to various causes is a minimally invasive procedures undertaken for enteral nutrition. The feature in our case is to pierce the front and rear walls of the ileum and stomach catheter PEG insertion of the posterior wall is continued for a year without symptoms of complications.

Case report: 2-year-old male patient with perinatal asphyxia secondary to cerebral palsy, mental motor retardation, epilepsy and microcephaly were followed by out patient diagnosis. Difficulty in swallowing, inabilityto feed and development of the three attached PEG by pediatric gastroenterology reasons is below the percentile. Patients were followed up regularly for one year and continued life without major problems. Patients who have continued to increase last month, vomiting, inability to feed and went to the pediatric gastroenterology out patient clinics with complaints of leakage around the PEG. Pediatric surgery clinic was held in check after endoscopic gastrostomy clinic directed to revision by the pediatric gastroenterology.

Exploration of the catheter in the small intestine performed by laparotomy was seen late by perforated front and rear walls. It was also found that endoscopic gastrostomy fitted posterior wall of the stomach. Ileal resection and anastomosis was performed and the catheter was deregulated. Then the catheter is removed and the stomach was fixed. After antireflux surgery was performed prior to the patient underwent gastrostomy stam.

Conclusion:Although there are many considered to be minimal surgery has its own complications. These are the separation of theabdominal wall of the stomach, ileal or colonic perforation, peritonitis, togetout of the head mushrooms in thestomach, gastro-colic fistula.Among them is the most important complications are these paration of the stomach from the abdominal wall and intestinal perforation and gastrocolic fistula.During endoscopy, when the stomach in adequate inflated, it can remain below the colon or ileum, and the catheter can pass via perforate them.

Discussion:PEG is a minimally invasive procedure, maintenance is easy and provides a permanent way to enteral feeding in children with disabilities. These can be seen as complications so can also be presented in morbidity and mortality. There are many studies evaluating complications occurred during the implementation PEG.

Perkütan endoskopik gastrostominin nadir bir komplikasyonu: trans-ileal perforasyon ve posterior duvar gastrostomisi

Giriş: Perkütan Endoskopik Gastrostomi (PEG), çeşitli nedenlere bağlı beslenme sorunu olan hastalara uzun süreli enteral beslenme desteği için uygulanan minimal invaziv bir girişimdir. Olgumuzun özelliği ise PEG kataterinin ileum ön ve arka duvarlarını delip geçmesi ve mide posterior duvardan takılması olup komplikasyonunun bir yıl boyunca semptom vermeden devam etmesidir.

Olgu sunumu: 2 yaşında erkek hasta, perinatal asfiksiye skonder serebral palsi , mental motor retardasyon , epilepsi ve mikrosefali tanıları ile poliklinikten takip edilmiş. Yutma güçlüğü, beslenememe ve gelişiminin 3 persentilin altında olması nedenlerinden dolayı pediatrik gastroenteroloji tarafından PEG takılmış. Hasta bir yıl süre ile düzenli olarak takip edilmiş ve major bir sorun yaşamadan hayatına devam etmiştir. Ancak son bir aydır artarak devam eden kusma, beslenememe ve PEG etrafından kaçak şikayetleri ile tekrar başvurmuş. Pediatrik gastroenteroloji kliniğinde yapılan kontrol endoskopi sonrasında çocuk cerrahisi kliniğine gastrostomi revizyonu için yönlendirildi. Laparotomi ile yapılan eksplorasyonda bir yıl önce takılan PEG kataterinin ince barsak ön arka duvarlarını trans ileal olarak perfore ederek geçtiği görüldü. Ayrıca mideposterior duvardan endoskopik olarak gastrostomi takıldığı tesbit edildi.İleal rezeksiyon ve anastomoz yapılarak PEG katateri serbestleştirildi. Daha sonra katater mideden çıkarılarak tamir edildi. Hastaya nissen antireflü cerrahi yapılarak stamm gastrostomi şeklinde revizyon yapıldı.

Bulgular: Minimal cerrahi girişim olarak kabul edilse de PEG’in kendine ait komplikasyonları da vardır. Bunlar; midenin karın duvarından ayrışması, ileal veya kolonikperforasyon, peritonit, mide içindeki mantar başın dışarıya çıkması, gastro-kolik fistül gelişimi, katater çıkarıldıktan sonra fistül traktusunun kapanmamasıdır. Bunlar arasında en önemli komplikasyonlar midenin karın duvarından ayrışması ve intestinal perforasyon ve gastrokolik fistül gelişimidir. Endoskopide midenin yetersiz şişirilmesi kolonun veya ileumun yukarda kalmasına ve dolayısıyla kataterin içinden geçirilmesine yol açabilir.

Tartışma: PEG minimal girişimsel olması, bakımının kolaylı­ğı ve engelli çocuklarda enteral beslenme için kalıcı bir yol sağlaması gibi avantajlara sahiptir. Ancak nadiren çeşitli komplikasyonlar ile karşılaşılmaktadır. Bu komplikasyonlar erken ve geç olarak görülebildiği gibi mortal ve morbit de seyredebilmektedir.
