Approach to prenatally diagnosed cervical mass with extrauterine intrapartum treatment (EXIT)

Aim: To present the case of a cervical mass, compressing the trachea during the prenatal period, and to which the extrauterine intrapartum treatment (EXIT) procedure was performed during the labour.

Case: The case of polyhydromnios and cervical mass, detected during the prenatal follow-ups, was evaluated together with the perinatology team during the intrauterine period. On the thirty-seventh gestational week, it was decided to perform a delivery together with the EXIT procedure by the team comprised of the perinatologist, anesthesiologist, newborn specialist and the pediatric surgeon. The general anesthesia was administered to the mother when the whole intraoperative was sterile and ready. The baby was delivered with cesarean section. The oropharyngeal intubation was performed to the baby at the level of the placenta by preserving the uteroplacental and placentofetal circulation and without clamping the cord. After the intubation, the stable patient, whose cord had been clamped, was transferred to the neonatal intensive care unit. A solid contiguous solid mass lesion (65x65x55 mm), which covered the right half of the neck, passed the middle line and which extended to the pterygopalatin fossa, was observed in the cervical CT. The mass was totally excised with the right cervical incision on the third postnatal day. The patient was extubated on the 6th postnatal day. The histopathologic examination was consistent with the immature teratoma. Since the immature component of the mass was less than 20% and the surgical margin was clean, the clinical follow-up of the patient without chemotherapy was decided during the oncology council. Conclusion: Since the EXIT procedure allows placental-fetal circulation, it is a safe treatment option for the cervical masses, detected during the prenatal period, which cause a serious airway obstruction. A multidisciplinary approach and planning are needed for this advanced perinatal resuscitation technique.

Prenatal tanılı servikal kitleye ekstrauterin intrapartum tedavi (EXİT) yöntemiyle yaklaşım

Amaç: Prenatal dönemde trakeaya bası yapan servikal kitle saptanan ve doğumda ekstrauterin intrapartum tedavi (EXIT) prosedürü uygulanan olgunun sunulmasıdır.

Olgu: Prenatal takiplerinde polihidromaniyos ve servikal kitle saptanan olgu intrauterin dönemde perinataloji ekibi ile birlikte değerlendirildi. Fetal MR’da boyun sağ yarımından kaynaklanan ve trakeada basıya yol açan kitle mevcuttu. Otuzyedinci gestasyonel haftada perinataloji, anestezi, yenidoğan ve çocuk cerrahisinden oluşan ekiple EXIT prosedürü ile doğumun gerçekleştirilmesine karar verildi. İntraoperatif tüm ekibin steril olarak hazır olduğu vakada anneye genel anestezi uygulandı. Sezaryen ile bebek doğurtuldu. Bebek uteroplasental ve plasentofetal dolaşım korunarak kord klemplenmeden plasenta seviyesinde orofarengeal olarak entübe edildi. Entübasyon sonrası kordu klemplenen ve stabil olan olgu yenidoğan yoğun bakım ünitesine alındı. Postnatal 2.günde yapılan servikal BT’de boyun sağ yarımını kaplayan orta hattı geçen pterigopalatin fossaya uzanan 65x65x55 mm, yaygın vasküler alanlar bulunan düzgün konturlu solid kitlesel lezyon saptandı. Postnatal 3.gün sağ servikal insizyon ile kitle total olarak eksize edildi. Olgu postnatal 6.günde ekstübe edildi. Histopatolojik inceleme immatür teratom ile uyumlu idi. Kitlenin immature komponentinin %20’den az olması ve cerrahi sınırın temiz olması nedeni ile onkoloji konseyinde kemoterapisiz klinik takip kararı verildi. Olgu postoperatif 7. ayda sorunsuz olarak izlenmektedir.

Sonuç: Prenatal dönemde saptanan ciddi havayolu tıkanıklığına yolaçan servikal kitlelerde EXIT prosedürü plesantofetal dolaşıma izin verdiği için güvenli bir tedavi seçeneğidir. Bu ileri perinatal resusitasyon tekniği için multidisipliner yaklaşım ve planlanma gerekmektedir.
