Giant Cervical Cystic Tumor with Severe Respiratory Distress: Case Report

AİM: In children, cervical congenital giant masses are very important because they cause respiratory distress. This is presented to emphasize the importance of neonatal patient's disease, which is caused by respiratory distress due to giant mass and can be difficult to treat.

MATERİAL AND METHODS: A male newborn who is urgently transferred to our clinic with severe respiratory distress. An urgent operation was performed to the patient with advanced respiratory distress and the cervical mass was removed totally.

RESULTS: Giant cervical masses may cause postpartum respiratory distress with the pressure they cause on the peripheral organs as well as they make the birth difficult. Because of this EXIT procedure can be performed for patients with antenatal diagnosis. This procedure was not performed in our patient and patient was born with caesarean section but it was seen that the giant mass (15x10x9cm size) was deviating the head to the left side and causing advanced respiratory distress. The patient was intubated difficultly and taken to the operation immediately. In the histopathological examination of the mass, which is totally removed during surgery, neuroblastoma or immature teratoma was considered firstly. However, considering the fact that the brother of the patient was followed by the embryonal tumor of the central nervous system, genetic examination was required for the differential diagnosis of the Ewing sarcoma. The follow-up and examination of the patient who transferred from our clinic to the clinic of oncology continues.

CONCLUSION: Cervical neck masses are important congenital anomalies that can cause fatal complications. Surgery to be done without delay for advanced examinations for treatment is life saving because it will eliminate the complications.

İleri derecede Solunum Sıkıntısı Oluşturan Servikal Dev Kistik Tümör: Olgu Sunumu

AMAÇ: Çocuklarda servikal doğumsal dev kitleler solunum sıkıntısı oluşturdukları için çok önemlidir. Dev kitle nedeniyle solunum sıkıntısı ile doğan ve zor entübe edilebilen yenidoğan hastamız hastalığın önemini vurgulamak için sunulmuştur.

GEREÇ VE YÖNTEM: Solunum sıkıntısı ile kliniğimize acil olarak getirilen erkek yenidoğan. İleri derecede solunum sıkıntısında olan hasta acil olarak ameliyata alınarak servikal kitle total olarak çıkarıldı.

BULGULAR: Dev servikal kitleler, çevre organlara yaptıkları bası ile postpartum solunum sıkıntısına yol açacakları gibi doğumu da zorlaştırabilirler. Antenatal tanılı hastalarda bu nedenle EXIT işlemi uygulanabilir. Sezeryan ile doğan hastamızda bu işlem uygulanmamış ancak postpartum başı sola deviye eden dev kitlenin (15x10x9cm boyutunda) ileri derecede solunum sıkıntısına yol açtığı görülmüştür. Bunun üzerine hemen zorlukla entübe edilen hasta acilen ameliyata alınmıştır. Ameliyatta total olarak çıkarılan kitlenin histopatolojik incelemesinde öncelikle nöroblastom veya immatür teratom düşünülmüştür. Ancak hastanın kardeşinin de santral sinir sistemi embriyonel tümörü tanısı ile izlemde olduğu göz önüne alınarak tümörün Ewing sarkomdan ayırıcı tanısı için genetik inceleme istenmiştir. Kliniğimizden Onkoloji kliniğine nakledilen hastanın izlemi ve incelemeleri sürmektedir.

SONUÇ: Servikal boyun kitleleri ölümcül komplikasyonlara yol açabilen önemli doğumsal anomalidir. Tedavi için ileri incelemeler ile zaman kaybetmeden yapılacak cerrahi girişim komplikasyonları ortadan kaldıracağı için yaşam kurtarıcıdır.
