The Evaluation of Effects of Neuregulin-1 on Endoplasmic Reticulum Stress, Apoptosis and Testiculer Damage of Torsioned/Detorsioned Rat Testis

Aim: Testicular torsion, more often seen in males aged 12-18 years, is an urological urgency that causes ischemia of the testis, requires rapid diagnosis and intervention. Treatment is detorsion of the testis. While testicular torsion causes ischemia, reperfusion following detorsion causes additional damage to the testis. The aim of this study is to investigate, whether Neuregulin-1, an endogenous polypeptide, is protective against ischemic reperfusion injury due to its effect on endoplasmic reticulum stress.

Method: 3-4 moths old 48 Wistar rats were divided into 5 groups: Control group, Long-term T/D group, Long term T / D + Neuregulin-1 group, Short-term T / D + Neuregulin-1 group and Short-term T / D group. Except the rats of the control group, all animals had a left inguinal incision from where the left testis was approached and was rotated 720 degrees anti-clockwise. After a waiting period of 2 hours the NRG-1 group rats were injected systemic NRG-1, the testis was detorsioned then and was left at neutral position. Groups with short T / D were sacrificed at 6th hour after detorsion, groups with long T / D were sacrificed at 24th hour after detorsion. Both testes were then evaluated with light microscopy and examined for GRP78, p-PERK, CHOP and caspase-12 levels immunohistochemically.

Findings: Ischemia and reperfusion injury induced in the left testis caused an increase in expression of GRP78, p-PERK and CHOP, which are UPR signal proteins activated by ER stress. With NRG1 administration it has been found that there is a significant decrease in the expression of these proteins. There was no difference in caspase-12 levels.

Conclusion: NRG-1 suppresses ER stress after T / D in testis cells. Further molecular level studies are required to assess which step of this complex system with a large number of pathways is involved. The usability of NRG1 for the prevention reperfusion injury has also to be assessed with more relevant clinical studies.

Neuregulin-1’in Sıçan Testislerinde Torsiyon/Detorsiyon Sonucu Oluşan Endoplazmik Retikulum Stresi, Apoptoz Ve Testiküler Hasar Üzerine Etkisinin Değerlendirilmesi

Amaç: Testis torsiyonu testiste ileri düzeyde iskemiye neden olan, sıklıkla 12-18 yaş arası erkeklerde görülebilen, akut seyirli hızlı teşhis ve tedavi gerektiren bir ürolojik acildir. Tedavisi cerrahi olarak testisin detorsiyone edilmesidir. Torsiyon esnasındaki iskemi kadar detorsiyon sonrası oluşan reperfüzyon da testiste hasara neden olmaktadır. Bu çalışmanın amacı endojen bir polipeptit olan Neuregulin-1’in endoplazmik retikulum stresi üzerindeki etkisi nedeniyle iskemi reperfüzyon hasarında koruyucu etkisi olup olmadığının araştırılmasıdır.

Yöntem: 3-4 aylık 48 erkek Wistar sıçan 5 gruba ayrıldı: Kontrol grubu, Kısa süreli torsiyon/detorsiyon (T/D) grubu, Uzun süreli T/D grubu, Kısa süreli T/D + Neuregulin-1 grubu, Uzun süreli T/D + Neuregulin-1 grubu. Kontrol grubu dışındaki sıçanlarda sol inguinal insizyon ile sol testis ve korda ulaşılıp, sol testisi 720 derece saat yönünün tersine çevrildi ve 2 saat süre ile beklendi. İki saatlik torsiyon süresi sonrası NRG-1 gruplarına detorsiyon öncesi sistemik NRG-1 enjeksiyonu yapıldı ve ardından sol testis detorsiyone edilerek nötral pozisyona getirildi. Kısa süreli T/D yapılan gruplar detorsiyon sonrası 6.saatte, uzun süreli T/D yapılan gruplar detorsiyon sonrası 24.saatte sakrifiye edildi. Ardından her iki testis ışık mikroskobi ve immunhistokimyasal inceleme için çıkarıldı. GRP78, p-PERK, CHOP ve kaspaz 12 değerleri ölçüldü.

Bulgular: Çalışmamızın bulgularına göre, sıçan sol testisinde oluşturulan iskemi ve reperfüzyon hasarı, ER stresi ile birlikte aktif hale geçen UPR sinyal proteinleri olan GRP78, p-PERK ve CHOP ekspresyonlarında artışa neden olmuştur. NRG1 uygulanması ile birlikte bu proteinlerin ekspresyonlarında anlamlı derecede azalış olduğu tespit edilmiştir. Kaspaz 12 düzeyinde ise farklılık görülmemiştir.

Sonuç: NRG-1 testis hücrelerinde T/D sonrasında ER stresini baskılamaktadır. Çok sayıda yolağı olan bu kompleks sistemi hangi basamağının etkilendiğini değerlendirmek ve sonrasında NRG1 uygulamasını klinik pratiğimizde yer almasını sağlamak için ileri moleküler çalışmalara ihtiyaç vardır.
