The role of prenatal diagnosis in duodenal obstruction

Background/Aim: The presence of severe cardiac malformations or down syndrome in the fetuses with duodenal atresia /obstruction determine the decision of termination of pregnancy. In this study the effect of prenatal diagnosis deciding on termination or breeding was searched.

Materials/Methods: The hospital records of patients who were operated with congenital duodenal obstruction diagnosis in our clinic were reviewed retrospectively, between 2004 and 2017. The patients were evaluated acording to their gestational features.

Results: 68 newborns (36 males, 32 females) with congenital duodenal obstruction were operated in the period of 13 years. The mean maternal age 28.1..4,8 years (18-40 years) . Sixty three of the patients (92.6%) had regular prenatal ultrasound. While 35 patients (51.4%) had signs of duodenal atresia/ obstruction and 41 patients (60.3%) had polyhydramniosis. The prenatal diagnosis age was 29.2…1.5 weeks (27-32 weeks). Associated abnormalities were detected in 37 patients (54.4%). Sixteen patients (23.5%) had congenital cardiac malformations and 13 patients (19.1%) had Down syndrome.

Three patients (4.4%) were born in 2004, 9 patients (13.2%) in 2005, 3 patients (4.4%) in 2006, 6 patients (8.8%) in 2007, 10 patients (14.7%) in 2008, 4 patients (5.9%) in 2009, 5 patients (7.4%) in 2010, 6 patients (8.8%) in 2011, 7 patients (10.2%) in 2012, 2 patients (2.9%) in 2013, 4 patients (5.9%) in 2014, 4 patients (5.9%) in 2015 and 4 patients (5.9%) in 2016.

Nine patients (13.2%) were born in january, 2 patients (2.9%) in february, 3 patients (4.4%) in march, 6 patients (8.8%) in april, 4 patients (5.9%) in may, 5 patients (7.4%) in june, 2 patients (2.9%) in july, 9 patients (13.2%) in august, 9 patients (13.2%) in september, 6 patients (8.8%) in october, 7 patients (10.3%) in november, 6 patients (8.8%) in december.

Conclusions: The prenatal diagnosis of patients with duodenal atresia/ obstruction after 27 weeks of gestational age and decrease in the number of cases by the year show that the termination can be the reason of early prenatal diagnosis and the associated diseases such as down syndrome and severe cardiac malformations don’t have effect on deciding termination. The incresed number of cases in january, august and september attracted attention.

Duodenal obstrüksiyonda prenatal tanılamanın rolü

Amaç:

Prenatal tanılı duodenal atrezi/obstrüksiyonda eşlik edebilecek ağır kardiak malformasyonlar veya Down Sendromu sıklıkla gebelik terminasyonu kararını belirlemektedir. Bu çalışmada; opere duodenal atrezi/obstrüksiyonlu yenidoğanlarda prenatal tanının terminasyon/doğum kararına etkisi araştırıldı.

Yöntem:

Ocak 2004-2017 arasında kliniğimizde duodenal obstrüksiyon/atrezi tanısı ile opere edilen yenidoğanların hastane kayıtları retrospektif olarak incelendi; hastaların gestasyonel özellikleri değerlendirildi.

Bulgular:

Onüç yılda 32’si(%47) kız, 36’sı(%53) erkek, toplam 68 yenidoğan duodenal obstrüksiyon/atrezi ön tanısı ile kliniğimizde opere edildi. Ortalama anne yaşı 28.1±4.8 yıl (18-40 yıl) olup 63’ünde(%92.6) düzenli Prenatal Ultrason yapılmıştı. 35’inde (%51.4) duodenal atrezi/obstrüksiyon bulguları, 41’inde (%60.3) polihidramniyoz mevcuttu. Prenatal tanı yaşı 29.2 ±1.5 hafta(27-32 hafta) idi. 16’sı(%23.5) konjenital kalp hastalığı, 13’ü(%19.1) Down Sendromu olmak üzere 37 hastada (%54.4) ek anomali mevcuttu.

Olguların 3’ü(% 4.4) 2004, 9’u(%13.2 ) 2005, 3’ü(%4.4 ) 2006, 6’sı(% 8.8 ) 2007, 10’u(% 14.7) 2008, 4’ü(% 5.9) 2009, 5’i(% 7.4 ) 2010, 6’sı(%8.8) 2011, 7’si(%10.2) 2012, 2’si(%2.9) 2013, 4’ü(%5.9) 2014, 4’ü(%5.9) 2015, 4’ü(%5.9) ise 2016 yılı doğumlu idi.

Olguların 9’u (%13.2) Ocak, 2’i (%2.9) Şubat, 3’ü (%4.4) Mart, 6’sı (%8.8) Nisan, 4’ü (%5.9) Mayıs, 5’i (%7.4 ) Haziran, 2’i (% 2.9) Temmuz, 9’u (%13.2) Ağustos, 9’u (%13.2) Eylül, 6’sı (%8.8) Ekim, 7’si (%10.3) Kasım, 6’sı (%8.8) ise Aralık doğumlu idi.

Sonuç:

Opere edilen duodenal atrezi/obstrüksiyonlu olguların 27.hafta sonrası tanılanmaları ve yıllara göre olgu sayısının giderek azalması erken prenatal tanılama ile gebelik terminasyonu yapıldığını ancak ailelerin terminasyon kararında Down Sendromu ya da ağır ek anomali eşlik etmesinin halen etkili olmadığını düşündürmektedir. Olgularının Ocak, Ağustos ve Eylül aylarında daha yüksek oranda görülmesi dikkat çekmiştir.
