Liver injuries due to firearm

Introduction: The liver is the largest volume solid organ in the body. Because of the blood flow from both the portal system and the systemic circulation, when liver injuries, there is a sudden and potentially life-threatening blood loss due to intrahepatic veins and / or vena cava inferior damage.Computed tomography is important for diagnosis. Subcapsular hematomas are followed by ultrasonography. The vital signs of the patient and the amount of transfusion required are parameters that make the decision of operation.

Methods: We retrospectively reviewed the patients who had liver injury due to firearm followed by Mustafa Kemal University pediatric surgeon's clinic.

Findings: Twenty patients with hepatic injury were examined. Three of the patients were female (15.7%) and 16 (84.3%) were male. The mean age of the patients was 7,8. Liver injury maked by blast effects due to exposure bombing in 7 patients , fragment injury in 13 patients.Liver suture was performed in 2 patients due to bleeding. One patient underwent segment resection. 2 patients were performed operation in Syria and put the compress into the abdomen because the bleeding could not be stopped. Six patients died due to head trauma and multiple trauma. No late complications such as hemobilia, bile peritonitis, ileus were observed in any of the patients.

Conclusion: Even in the case of liver injuries with firearms, in the majority of the cases don't need any operation. The main purpose in operation is to stop the bleeding. Suturing in the bleeding zone may require segmentectomy in rare cases.

Karaciğerin ateşli silahla yaralanmaları

Giriş: Karaciğer vücutta en büyük hacimli solid organdır. Hem portal sistemden hem de sistemik dolaşımdan gelen kan akımına sahip olduğundan yaralanmalarında intrahepatik damarlar ve/ veya arkasında bulunan vena kava inferior hasarıyla hayatı tehdit edici ani ve büyük boyutlarda kan kaybı olur. Tanıda bilgisayarlı tomografi önemlidir. Subkapsüler hematomların takibinde ultrasonografi kullanılır. Hastanın vital bulguları ve ihtiyaç duyulan transfüzyon miktarı operasyon kararı verdirten parametrelerdir.

Yöntem: Mustafa Kemal Üniversitesi çocuk cerrahisi kliniğinde takip edilen ateşli silahla karaciğer yaralanması olan hastalar retrospektif olarak incelendi.

Bulgular: Karaciğer yaralanması olan 20 hasta incelendi.Hastaların 3'ü kız (%15.7) 16'si erkekti(%84.3). Hastaların yaş ortalaması 7.8 idi. 7 hastada bombanın blast etkisiyle, 13 hastada şarapnel yaralanması mevcuttu. 2 hastada kanama nedeniyle karaciğer sütürasyonu yapıldı. Bir hastada segment rezeksiyonu yapıldı. 2 hastaya Suriye'de opere edilip kanamanın durdurulamaması nedeniyle batın içine kompres konulmuş idi. 6 hasta kafa travması ve multipl travma nedeniyle ex oldu. Hastaların hiçbirinde hemobili, safra peritoniti,ileus gibi geç dönem komplikasyonlar izlenmedi.

Sonuç: Ateşli silahla karaciğer yaralanmalarında bile vakaların çoğunda operasyon ihtiyacı hastaların az bir kısmında gerekmektedir. Operasyonda ana amaç kanamanın durdurulmasıdır. Kanayan bölgede sütürasyon, nadiren de olsa segmentektomi gerekebilir.
