APPROACH TO BLADDER TRAUMA IN A CASE OF BLADDER CONTUSION DUE TO NON-IATROGENIC TRAUMA

Trauma-related injuries may be a life-threatening health problem. In the United States, one out of every 14 deaths is due to a trauma.1 Traumas are the leading cause of death in children and young age groups. The rate of urogenital system involvement in trauma is 10% and the most commonly affected organ is the kidney (15%). Urogenital traumas are often concomitant with intraabdominal organ traumas. Therefore, a well-organized team is needed for the approach to a patient with trauma and the surgeon is an important part of this team. Bladder injury is the result of an external trauma or an iatrogenic injury. It occurs most commonly as a result of a blunt trauma. Very rarely, spontaneous bladder injuries can also be encountered.1 A trauma is classified as an iatrogenic or a non-iatrogenic injury according to the cause of the trauma. Blunt traumas are the cause in about 60-90% of all non-iatrogenic bladder injuries. Macroscopic hematuria is one of the main symptoms of the bladder injury. But rarely, it should be taken into consideration that bladder injury may emerge without macroscopic or microscopic hematuria. One of the most prominent complaints of the patients is the absence of micturition sensation and the inability to urinate spontaneously. CT cystography evaluation during the CT imaging process would be a proper approach regarding the efficient use of time, cost and early diagnosis in patients with the suspicion of the bladder injury, as most of them are trauma patients. During the clinical follow-up, one should watch out for the emergence of hematuria and a possible sepsis. The recovery is followed with a cystography evaluation carried out after a 7-to-14-day follow-up with catheterization. Antibiotic prophylaxis is initiated. The objective of this case report was to remind the importance of the triphasic CT imaging in the traumas encountered in the childhood accompanied with the suprapubic tenderness and hematuria in order not to overlook the urogenital system traumas particularly bladder traumas and the importance of the treatment protocol in the bladder contusion.

ÇOCUKLARDA İATROJENİK OLMAYAN TRAVMAYA BAĞLI GELİŞEN MESANE KONTÜZYONU OLGUSU EŞLİĞİNDE MESANE TRAVMALARINA YAKLAŞIM

Travmaya bağlı hasarlar hayatı tehdit edebilen bir sağlık problemidir. ABD’ de her 14 ölümden biri travmalar ne­deni ile olmaktadır.1 Travmalar çocuk ve genç yaş gru­bunda ölümlerin en önde gelen nedenidir. Travmada üro­genital sistemin etkilenme oranı %10’dur ve en sık etkile­nen organ ise böbrektir (%15). Ürogenital travmalar çoğu kez karın içi organ travmaları ile birlikte olur. Bu neden­le travmalı hastaya yaklaşım, iyi organize edilmiş bir ta­kım ile yapılır ve cerrah bu takımın önemli bir parçasıdır. Mesane yaralanması eksternal travma sonucu veya iyat­rojenik yaralanma şeklinde görülür. En sık künt travmalar şeklinde meydana gelmektedir. Çok nadir de olsa, spon­tan mesane yaralanmaları da görülebilmektedir.1 Travmanın oluşum şekline göre iatrojenik ve iatrojenik ol­mayan yaralanmalar olmak üzere ikiye ayrılır. Künt trav­malar iatrojenik olmayan mesane travmasının yaklaşık %60-90’ını oluşturmaktadır. Makroskopik hematüri mesane yaralanmasının ana belir­tilerindendir. Ama nadir de olsa makroskopik veya mik­roskopik hematüri olmadan da mesane yaralanması ola­bileceği unutulmamalıdır. Hastaların en belirgin bir başka şikayeti idrar hislerinin olmaması ve spontan idrar yapamamalarıdır. Mesane yaralanması düşünülen hastaların çoğunun trav­ma hastası olması nedeni ile BT ile görüntüleme esna­sında BT sistografi yapılması zaman, maliyet ve hızlı tanı için doğru yaklaşım olacaktır. Klinik takip sırasında hematüriye ve gelişebilecek sepsise dikkat etmek gereklidir. 7-14 gün sondalı takip sonra­sı çekilen sistografi ile iyileşme takip edilir. Antibiyotik profilaksisi başlanır. Bu olgu sunumu ile çocuk yaş grubu travma olgularında suprapubik bölgede hassasiyet ve hematüri eşlik eden olgularda ürogenital sistem travmalarının özellikle mesane travmalarının atlanmaması için trifazik BT çekilmesinin önemi ve mesane kontüzyonun tedavi protokolunu yeniden hatırlatmayı amaçladık.
